阿尔茨海默病患者静息态局部一致性与体素镜像同伦连接研究 被引量:8

Regional Homogeneity of Alzheimer's Disease at Resting-state and Voxel-mirrored Homotopic Connectivity

摘要 目的探讨阿尔茨海默病(AD)患者静息状态下局部自发脑活动及两半球相同脑区之间功能连接的改变。资料与方法纳入兰州大学第一医院神经内科确诊为AD的24例患者,同时招募24例正常老年人作为对照组。受试者行3.0T磁共振静息态BOLD扫描。对所有受试者扫描图像资料行局部一致性(ReHo)和镜像对侧同伦连接(VMHC)分析,通过统计分析ReHo异常功能脑区域及运用VMHC分析该异常区域半球间功能连接状态。结果与对照组相比,AD组双侧额内侧上回、眶额中下回、眶额内侧回、前中扣带回、尾状核、右侧海马、右侧岛回、右侧杏仁核、右侧额中回及左侧颞上、下回Re Ho值增高,双侧枕中回、枕上回、距状皮层、楔前叶、后扣带回、中央前后回、角回、额下回岛盖、左侧顶下小叶、左侧额中回、左侧辅助运动区及右侧额上回ReHo值减低(Voxel水平,P<0.05,TFCE校正);AD组双侧嗅回、额内侧上回、眶额中上回、眶额内侧回、前扣带回、尾状核及岛回的VMHC值增高;双侧距状皮层、颞中回、枕中回VMHC值减低(Voxel水平,P<0.05,TFCE校正)。结论 AD患者较对照组Re Ho及VMHC明显异常脑区主要集中于默认网络核心脑区,两半球等位体素之间功能连接的异常可能与局部脑功能活动有关。 Purpose To explore the change of regional spontaneous cerebration at resting state and functional connection between the same encephalic region of cerebral hemisphere in patients with Alzheimer's disease(AD).Materials and Methods Twenty-four patients diagnosed as AD in Department of Neurology,the First Hospital of Lanzhou University were included in the study,and 24 cases were enrolled in normal elderly control group.Subjects underwent 3.0T magnetic resonance resting-state BOLD scan.Scanned image data of all subjects performed regional homogeneity(ReHo)and voxel-mirrored homotopic connectivity(VMHC)analysis.The encephalic region with abnormal ReHo function was statistically analyzed and the functional connectivity between cerebral hemisphere of the anomalous region was analyzed using VMHC.Results Compared with the control group,ReHo values of bilateral superior frontal gyrus,middle and inferior orbital frontal gyrus,orbital frontal medial gyrus,anterior and middle cingulate gyrus,caudate nucleus,right hippocampus,right gyri insulae,right amygdaloid nucleus,right middle frontal gyrus and left superior and inferior temporal gyrus in the AD group increased,and ReHo values of bilateral middle occipital gyrus,superior occipital gyrus,calcarine cortex,precuneus,posterior cingulate gyrus,central anterior-posterior gyrus,angular gyrus,gyrus frontalis inferior insulae operculum,left inferior parietal lobule,left middle frontal gyrus,left supplementary motor area,and right superior frontal gyrus were reduced(Voxel level,P<0.05,TFCE correction);VMHC values of bilateral olfactory gyrus,medial superior gyrus,middle and superior orbital frontal gyrus,orbital frontal medial gyrus,anterior cingulate gyrus,caudate nucleus and gyri insulae were increased in the AD group;VMHC values of bilateral calcarine cortex,middle temporal gyrus,and middle occipital gyrus were reduced(Voxel level,P<0.05,TFCE correction).Conclusion Compared with the control group,encephalic regions with apparent ReHo and VMHC abnormality in AD patients are mainly concentrated in default network core encephalic region.The abnormal function connection between cerebral hemisphere allelic voxel may be related to regional brain functional activities.
