两种不同方法处理去甲肾上腺素药物外渗的效果观察

Comparison of Two Methods for Managing the Norepinephrine Extravasation

目的探讨周围静脉输注去甲肾上腺素外渗的安全护理方法。方法便利抽样选择2015年10月至2017年10月福州总医院神经外科使用去甲肾上腺素发生不同程度外渗患者40例为研究对象,按时间顺序将2015年10月至2016年8月发生去甲肾上腺素外渗24例患者为对照组,2016年9月至2017年10月发生去甲肾上腺素外渗的16例患者为观察组。对照组采用传统方法50%硫酸镁湿敷,观察组采用酚妥拉明封闭及湿热敷并联合水胶体敷料及湿润烧伤膏外涂,观察两组患者的治疗效果及起效所需时间。结果观察组患者的疗效优于对照组,起效所需时间少于对照组,差异均有统计学意义(均P<0.05)。结论早期使用酚妥拉明封闭及湿热敷并联合使用水胶体敷料及湿润烧伤膏,对于去甲肾上腺素外渗患者的治疗效果显著,并可加快外渗组织的愈合时间。

Objective To explore the nursing methods of treating extravasation caused by peripheral intravenous infusion of norepinephrine.Methods By convenience sampling,40 patients with varying degrees of extravasation of norepinephrine were selected from October 2015 to October 2017.Patients were divided into two groups in chronological order for retrospective study.Patients who had extravasation between October 2015 and August 2016 were selected in the control group(24 cases),and treated with a traditional method using 50%magnesium sulfate for wet dressing;while patients who had extravasation between September 2016 and October 2017 were named as observation group(16 cases),and treated with phentolamine for nerve block and applied with moist heat,combined with using of hydrocolloid dressings.The differences between two methods were observed and compared.Results The observation group has better curative effect than the control group,and the time needed to take effect was less than that of the control group,the differences were statistically significant(all P<0.05).Conclusions Using phentolamine for nerve block and applied with moist heat,combined with Comfeel transparent dressing and burn ointment,has a significant therapeutic effect on norepinephrine extravasation.
