舌侧与唇侧固定矫治对牙周组织影响的对比研究

Comparative study on the effects of lingual and labial orthodontics on periodontal tissues

目的通过与唇侧矫治对比,观察舌侧矫治对牙周健康的影响。方法选取30名患者,分别接受舌侧组和唇侧组固定矫治。在矫治前、矫治第3,6个月,对16,11,31,46位点进行牙周袋深度(PD)、牙龈指数(GI)和探诊出血指数(bleeding index,BI)的检查。在矫治第3,6个月时,使用无菌纸尖采集龈沟内细菌,实时荧光定量PCR检测放线伴放线杆菌、牙龈卟啉单胞菌、中间普氏菌、福赛斯坦纳菌和齿垢密螺旋体含量。结果在矫治第3,6个月时两组的PD、GI和BI值与矫治前比较均升高(P <0. 05);且在第3和第6个月时舌侧组PD、GI和BI均明显大于唇侧组(P <0. 05)。在矫治第3,6个月时舌侧组中除放线伴放线杆菌外的其他细菌数量均高于唇侧组(P <0. 05)。结论与唇侧固定矫治相比,舌侧固定矫治效果影响牙周组织的清洁,舌侧固定矫治时要注意口腔健康的维护。

Objective To compare the effect of lingual orthodontics versus labial orthodontics on periodontal lesions.Methods Thirty patients were treated by lingual(lingual group)and labial(labial group)orthodontics,respectively.The depth of periodontal pocket(PD),gingival index(GI)and bleeding index(BI)were examined at sites 16,11,31 and 46 before and three,six months after treatment.After treatment for 3,6 months,sterile paper tips were used to collect bacteria from gingival sulcus,and real time fluorescence quantitative PCR was used to detect the contents of Actinobacillus actinomycetemcomitans,Porphyromonas gingivalis,Prevotella interme-dia,Tannerella forsythia and Treponema denticola.Results After treatment for 3,6 months,the PD,GI and BI values increased in the two groups(P<0.05).After treatment for 3,6 months,the PD,GI and BI in lingual group were significantly larger than those in labial group(P<0.05).After treatment for 3,6 months,the number of bacteria in lingual group was higher than that in labial group except for Actinobacillus actinomycetemcomitans(P<0.05).Conclusion Compared with labial orthodontic treatment,lingual orthodontic affects periodontal tissue cleanliness,thus in lingual fixation,oral health maintenance should be paid attention to.
