尼可地尔对缺血性心肌病心力衰竭患者血清CPP、GDF-15表达的影响

Effect of nicorandil on serum CPP and GDF-15 expression in ischemic cardiomyopathy patients with heart failure

目的研究尼可地尔对缺血性心肌病心力衰竭患者血清和肽素(copeptin,CPP)、血浆分化生长因子-15(GDF-15)表达的影响。方法选取2015年2月至2017年2月在我院治疗的缺血性心肌病心力衰竭患者80例,随机分为对照组和研究组,每组患者40例,对照组患者采取对症常规治疗,研究组患者在对症常规治疗的基础上加用尼可地尔片,两组患者均治疗6个月,观察两组患者治疗效果,CPP、GDF-15表达,心脏功能以及不良反应之间的差异。结果研究组治疗效果(97. 50%)明显优于对照组(80. 00%),患者的左心室射血分数(t=11. 531,P <0. 05),左心室收缩末期内径(t=9. 459,P <0. 05),左心室舒张末期内径(t=4. 849,P <0. 05)以及6 min步行距离(t=4. 727,P <0. 05)都有了明显改善,血清CPP表达水平明显下降,GDF-15表达明显上升。结论尼可地尔联合基础治疗对于缺血性心肌病心力衰竭患者的疗效明显,血液中和肽素水平明显下降,血浆分化生长因子-15水平明显上升,对于患者的预后具有积极的作用,同时,不良反应率与基础治疗之间相似,值得在临床中进行推广。

Objective:To observe the effect of nicorandil combined with conventional treatment of heart failure in ischemic cardiomyopathy patients with serum CPP,GDF-15 expression.Methods:Eighty cases of heart failure were collected in our hospital from February 2015 to 2017,and they were divided into two groups according to the random number table:the observation group and the control group,with 40 cases in each group.The patients in the control group were treated with routine treatment.The patients in the study group were treated with Nicorandil Tablets on the basis of routine treatment.The two groups were treated for 6 months.The treatment effect of the two groups,the expressions of CPP and GDF-15,the cardiac function and the difference between the adverse reactions were observed.Result:The treatment effect of the study group(97.50%)was significantly higher than that of the control group(80%),the left ventricular ejection fraction(t=11.531,P<0.05),the end systolic diameter of the left ventricle(t=9.459,P<0.05),the size of the left ventricular end diastolic diameter(t=4.849,P<0.05)and the 6 minute walking distance(t=4.727,P<0.05).The expression of CPP in serum decreased significantly,and the expression of GDF-15 increased significantly.Conclusion:Nicorandil combined with basic treatment is effective for patients with ischemic cardiomyopathy heart failure significantly,blood and peptide levels decreased significantly and plasma differentiation growth factor-15 levels increase obviously.It has a positive effect on the prognosis of patients.At the same time,there is no statistical difference between the rate of adverse reactions and basic treatment,which is worthy of promotion in clinical practice.
