临床心灵关怀对恶性肿瘤患者化疗期间癌因性疲乏与生命质量的影响

Effect of clinical emotional support on cancer-related fatigue and quality of life during chemotherapy in patients with malignant tumors

目的探讨临床心灵关怀对恶性肿瘤患者化疗期间癌因性疲乏与生命质量的影响。方法选取2015年4月至2017年3月肿瘤内科接受化疗的204例恶性肿瘤患者为研究对象,将其随机等分为研究组和对照组,对照组予以常规护理干预,研究组在对照组基础上联合临床心灵关怀干预。比较两组患者化疗期间癌因性疲乏程度量表(BFI)、治疗信心量表(HHI)、睡眠质量量表(PSQI)、生命质量量表(FACT-G)、护理满意度(NSNS量表)及医患关系。结果化疗期间,两组患者均无转院、死亡、失访或终止治疗病例。研究组BFI,PSQI及医患关系评分均明显低于对照组(P <0.05),HHI,FACT-G评分及护理满意度明显高于对照组(P <0.05)。结论化疗期间给予恶性肿瘤患者临床心灵关怀干预,能有效改善其癌因性疲乏状态,提高患者睡眠质量,增强治疗信心,建立和谐互信的医患关系。

Objective To investigate the effect of clinical emotional support on cancer-related fatigue and quality of life during chemotherapy in patients with malignant tumors.Methods To compare cancer-related brief fatigue inventory(BFI),Herth hope index(HHI),Pittsburgh sleep quality index(PSQI),functional assessment of cancer therapy-general(FACT-G),Newcastle satisfaction with nursing scales(NSNS)and doctor-patient relationship,204 cases of patients,who underwent chemotherapy in medical oncology from April 2015 to March 2017,were observed,and randomly divided into study group and control group with equal members.The control group adopted routine nursing intervention,while the study group adopted clinical emotional support intervention on the basis of the control group.Results During chemotherapy,no patients in both groups were transferred to hospital,died,lost or terminated treatment.The scores of BFI,PSQI and doctor-patient relationship in the study group were significantly lower than those in the control group(P<0.05).The HHI,FACT-G score and nursing satisfaction were significantly higher than those in control group(P<0.05).Conclusion During chemotherapy,the application of clinical emotional support intervention on malignant tumor patients could effectively improve their cancer-induced fatigue and sleep quality,enhance their confidence in treatment,and establish a harmonious and mutual trust relationship between doctors and patients.
