心电图QRS波群时限与急性心力衰竭患者预后的相关性

Correlation between Electrocardiogram QRS Wave Duration and Prognosis of Patients with Acute Heart Failure

目的:探讨心电图QRS波群时限与急性心力衰竭患者预后的相关性。方法:选取2013年1月-2016年12月本院收治的急性心力衰竭患者201例,根据其心电图QRS波群时限值分成观察组110例和对照组91例,观察组QRS值≥120 ms,对照组QRS<120 ms。患者出院时均行心功能、左室射血分数(LVEF)值检查以及追踪患者1年内预后情况,包括复发或死亡病例。结果:两组患者在冠心病和扩张型心肌病的发生率相比,差异有统计学意义(P<0.01);对照组患者Killip分级Ⅰ级发生率明显高于观察组,而Ⅱ、Ⅳ、Ⅳ级的发生率均较观察组低,差异有统计学意义(P<0.05);观察组患者LVEF值低于对照组,但1年内终点事件发生率33.64%高于对照组,且期间复发病率高于对照组,差异有统计学意义(P<0.05)。结论:心电图QRS时限值超过120 ms的患者其LVEF值降低,且心血管终点事件发生率升高,急性心力衰竭患者心电图QRS时限延长其预后差,因此,临床应重视心电图QRS时限对急性心力衰竭患者预后的评估价值。

Objective:To explore the correlation between the time of electrocardiographic QRS wave and the prognosis of patients with acute heart failure.Method:201 patients with acute heart failure treated in our hospital from January 2013 to December 2016 were divided into observation group of 110 cases and control group of 91 cases according to the time limit of QRS wave.The QRS value of the observation group was more than 120 ms,and the control group was QRS<120 ms.At the time of discharge,cardiac function,left ventricular ejection fraction(LVEF)and prognosis in 1 year were recorded,including relapse or death.Result:There was a significant difference in the incidence of coronary heart disease and dilated cardiomyopathy in the two groups(P<0.01);the incidence of Killip gradeⅠin the control group was significantly higher than that in the observation group,while the incidence of gradeⅡ,ⅣandⅣwere lower than those of the observation group(P<0.05);the LVEF value of the patients in the observation group was lower than the control group,the difference was statistically significant(P<0.05);but the incidence of endpoint events in 1 year of observation group was 33.64%higher than that of the control group,and the recurrence rate was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion:The LVEF value of patients with QRS time limit of electrocardiogram(ECG)is lower than that of 120 ms,and the incidence of cardiovascular endpoint events is increased.The QRS time limit of ECG in patients with acute heart failure is poor.Therefore,the evaluation value of the QRS time limit on the prognosis of patients with acute heart failure is valued.
