中性粒细胞/淋巴细胞与血小板/淋巴细胞比值对痛风疾病活动及治疗后病情的评估

Assessment of neutrophil-lymphocyte ratio and platelet-lymphocyte ratio to disease activity and post-treatment conditions in gout patients

目的评估外周血中性粒细胞/淋巴细胞比率(NLR)、血小板/淋巴细胞(PLR)比率与痛风疾病活动及治疗后病情的相关性。方法收集于扬州大学附属苏北人民医院就诊的痛风患者112例及119例健康体检者临床资料,计算其NLR和PLR数值,比较两组NLR和PLR的差异,分析NLR和PLR与痛风患者临床因素的相关性及治疗前后的差异;将患者临床表现及相关实验室指标(血沉、C反应蛋白)作为疾病活动,绘制ROC曲线评价NLR和PLR对疾病活动的作用。结果痛风患者NLR和PLR水平高于健康对照组;痛风患者的NLR和PLR值与血沉、C反应蛋白呈正相关,与痛风患者的血尿酸水平、尿酸盐沉积量、家族史、痛风石无显著相关性;痛风患者治疗后的NLR和PLR水平较治疗前明显减低;ROC曲线分析表明NLR对评估病情活动性的灵敏度较高。结论 NLR和PLR与痛风患者病情相关,对痛风患者疾病活动及治疗后病情的评估有一定的价值。

Objective To evaluate the correlation of neutrophil-lymphocyte ratio(NLR)and platelet-lymphocyte ratio(PLR)with the disease activity and post-treatment conditions in gout patients.Methods Clini-cal data of 112 patients with gout and 119 healthy controls were collected.NLR and PLR were calculated and statis-tical differences of NLR and PLR between two groups were analyzed through non-parametric test.Correlation of NLR and PLR with clinical features such as ESR,CRP,serum uric acid,the family history,the tophi and the concentration of the uric crystal deposition in patients with gout were analyzed by Spearman.Comparisons of NLR and PLR before and after treatment of gout were analyzed by Paired Samples Test;According to the clinical mani-festations and related laboratory indicators(blood sedimentation,c-reaction protein)in patients with gout,the ROC curve was drawn to evaluate whether NLR or PLR could reflect disease activity in gout.Results the levels of NLR and PLR in gout patients were higher than those in healthy controls.Both NLR and PLR were positively corre-lated with ESR and CRP in patients with gout,whereas there was no correlation of NLR and PLR with the level of serum uric acid,the family history,the tophi and the concentration of the deposition of the uric crystal in gout pa-tients.Furthermore,the data also showed that NLR and PLR were significantly decreased after treatment when com-pared with those before treatment.In addition,the ROC curve showed that the more sensitivity of NLR to assess the disease activity of the patients with gout.Conclusion Both NLR and PLR were correlated with gout severity and may be helpful to assess the disease activity and to evaluate the treatment of gout patients.
