乌梅丸加减治疗抗精神病药物所致锥体外系反应的疗效评价

Evaluation of the treatment of extrapyramidal reaction induced by antipsychotics by modified Wumeiwan

目的:探讨乌梅丸加减治疗抗精神病药物所致锥体外系反应的疗效。方法:选自本院于2016年3月—2018年3月期间收治的抗精神病药物所致锥体外系反应患者90例,依据随机数字表法随机分为观察组45例与对照组45例,对照组于餐后口服苯海索片,2 mg/次,1～2次/d;观察组在对照组基础上结合乌梅丸加减治疗,1剂/d,以水600 ml,煎取200 ml,适温服,2次/d。两组疗程均为4周。比较两组临床疗效、治疗前后RESES总分、记时运动试验和10 m折返运动试验变化及不良反应发生情况。结果:观察组总有效率(100. 00%)高于对照组(84. 45%)(P <0. 05);两组治疗后RESES总分较治疗前降低(P <0. 05),观察组治疗后RESES总分低于对照组(P <0. 05);两组治疗后记时运动试验较治疗前延长(P <0. 05),观察组治疗后记时运动试验长于对照组(P <0. 05);两组治疗后起身时间较治疗前降低无明显变化(P>0. 05),而治疗后10 m行走时间和转弯时间较治疗前缩短(P <0. 05),观察组治疗后10 m行走时间和转弯时间快于对照组(P <0. 05)。观察组不良反应发生率(4. 44%)低于对照组(33. 33%)(P <0. 05)。结论:乌梅丸加减治疗抗精神病药物所致锥体外系反应的疗效显著,值得临床借鉴。

Objective:To investigate the efficacy of modified Wumeiwan in the treatment of extrapyramidal reaction induced by antipsychotic drugs.Methods:Ninety patients with extrapyramidal reaction induced by antipsychotics were randomly divided into observation group(45 cases)and control group(45 cases).The control group received benzepram tablets,and the observation group was treated with the addition and subtraction of Wumeiwan on the basis of the control group.Both groups were treated for 4 weeks.The therapeutic effect,total RESES score,time-keeping exercise test and 10-meter reentry exercise test before and after treatment,and adverse reactions were compared between the two groups.Results:The total effective rate of the observation group(100%)was higher than that of the control group(84.45%)(P<0.05).After treatment,the total RESES scores of the two groups were lower than those before treatment(P<0.05);after treatment,the total RESES scores of the observation group were lower than those of the control group(P<0.05).The time-keeping exercise test of the two groups after treatment was longer than that before treatment(P<0.05);the time-keeping exercise test of the observation group after treatment was longer than that of the control group(P<0.05).The time of walking and turning 10 meters after treatment was shorter than that before treatment(P<0.05),and the time of walking and turning 10 meters after treatment was shorter than that before treatment(P<0.05).The bending time was faster than that of the control group(P<0.05).The incidence of adverse reactions in the observation group(4.44%)was lower than that in the control group(33.33%)(P<0.05).Conclusion:Modified Wumeiwan is effective in the treatment of extrapyramidal reactions induced by antipsychotics,which is worthy of clinical reference.
