结直肠癌中IGF-1、IGFBP-3、MMP-14的表达及意义 被引量:4

Expression of IGF-1,IGFBP-3 and MMP-14 and their clinical significance in colorectal cancer

摘要 目的探讨胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)、胰岛素样生长因子结合蛋白-3(insulin-like growth factor binding protein-3,IGFBP-3)及基质金属蛋白酶-14(matrix metalloproteinases-14,MMP-14)在结直肠癌(colorectal cancer,CRC)中的表达及临床意义。方法采用免疫组化MaxVision法检测70例CRC、30例结直肠腺瘤(colorectal adenoma,CRA)及20例正常肠黏膜组织(对照组)中IGF-1、IGFBP-3及MMP-14的蛋白表达,并进行统计学分析。结果 IGF-1、MMP-14蛋白在CRC组织中的表达高于CRA、对照组,且与CRC浸润深度、有无淋巴结转移及临床分期有关(P <0. 05); IGFBP-3蛋白在CRC组织中的表达低于CRA、对照组,与CRC有无淋巴结转移及肿瘤分期有关(P <0. 05)。IGF-1、MMP-14与IGFBP-3蛋白表达呈负相关关系(r=-0. 804/-0. 460,P <0. 01),IGF-1与MMP-14呈正相关关系(r=0. 443,P <0. 05)。结论 IGF-1、IGFBP-3、MMP-14与CRC的发生、发展密切相关,联合检测三者有助于CRC的诊断及预后。 Purpose To investigate the expression of insulin-like growth factor-1(IGF-1),insulin-like growth factor binding protein-3(IGFBP-3)and matrix metalloproteinases-14(MMP-14)in colorectal cancer(CRC),and to explore their relationship with clinical and pathologic factors.Methods 70 cases of CRC,30 cases of colorectal adenoma(CRA)and 20 cases of normal colorectal mucosa were selected.Immunohistochemical MaxVision method was used to detect the expression of IGF-1,IGFBP-3 and MMP-14 protein and their correlaton with clinical pathological features.These data were statistically analyzed.Results In the CRC group,the expression level of IGF-1,MMP-14 protein was significantly higher than that of colorectal adenoma and colorectal mucosa.But the positive rate of IGFBP-3 protein was lower.There was negative correlation between the expression of IGF-1,MMP-14 and IGFBP-3(r=-0.804/-0.460,P<0.01),but positive correlation between IGF-1 and MMP-14(r=0.443,P<0.05).Conclusion In the development and progression of CRC,insulin-like growth factor(IGFs)and MMPs play an irreplaceable role,and so combined detection of IGF-1,IGFBP-3 and MMP-14 has of important significance in the early diagnosis and treatment of CRC.
