胶原蛋白止血海绵在猪肝脏出血模型中止血效果研究 被引量:2

摘要 目的通过比较两组胶原蛋白止血海绵和普通纱布在猪的肝脏创伤模型中的止血时间、出血量和局部组织反应,为可吸收止血材料在动物试验中的评价提供有效出血创伤模型,同时为胶原蛋白止血海绵的临床应用提供参考依据。方法将18头实验用小型猪(雌性未孕,体重30~40 kg)随机分为A、B、C三组(A组为胶原蛋白海绵组,B组为医用胶原蛋白海绵组,C组为普通纱布组),每头猪建立肝脏出血模型,定量检测胶原蛋白止血海绵和普通纱布的止血时间和出血量,并在8周后,解剖动物观察创面血性荚膜形成情况,取创伤部位肝脏组织进行HE染色,进行病理组织学观察炎症反应情况和创口愈合情况等。结果 A、B、C三组止血时间分别为(215.00±69.50)、(205.00±48.06)和(480.00±90.99)s,出血量分别为(3.08±1.65)、(2.71±1.34)和(15.77±3.58)g,A、B两组和C组比较止血时间和出血量均有显著性统计学差异(P<0.05)。病理观察结果表明所有动物肝脏创面无感染性炎症反应,未见血性荚膜,损伤创口愈合良好。结论猪的肝脏出血模型能有效评价止血材料的有效性,在本模型中胶原蛋白止血海绵和普通纱布相比有明显的止血作用。 Objective By comparing the hemostasis time,bleeding volume and local tissue reaction of two groups of collagen hemostatic sponge and common gauze in the liver trauma model of pigs,an effective bleeding wound model was established for the evaluation of absorbable hemostatic materials in animal experiments.At the same time,it provides reference for the clinical application of collagen hemostatic sponge.Methods Eighteen experimental miniature pigs(female without pregnant,body weight 30~40 kg)were randomly divided into three groups(group A:collagen sponge group;group B:medical collagen sponge group;group C:normal gauze group).A model of hepatic hemorrhage was established in each pig.The hemostatic time and amount of bleeding of collagen hemostatic sponge and gauze were measured quantitatively.After 8 weeks,the formation of blood capsule of wound was observed in anatomical animals,and HE staining was used to observe the inflammatory reaction and wound healing in the injured liver tissue.Results The hemostasis time of group A,B and C were(215.00±69.50),(205.00±48.06)and(480.00±90.99)s,respectively.The hemorrhage volume was(3.08+1.65),(2.71+1.34)and(15.77+3.58)g,respectively.There were significant statistical differences in hemostasis time and hemorrhage volume among group A,B and group C(P<0.05).The pathological observation showed that there was no inflammatory reaction in all animal liver wounds,no bloody capsule was found,and the wound healed well.Conclusion Porcine liver hemorrhage model can effectively evaluate the effectiveness of hemostatic materials in this model.Collagen hemostatic sponge has a significant hemostatic effect compared with the normal gauze.
