低分子肝素钙不同注射方式治疗带蒂皮瓣静脉危象的效果对比

Effect comparison of Low Molecular Weight Heparin Calcium with different injection methods in the treatment of venous crisis of pedicle flap

目的对比低分子肝素钙不同注射方式治疗带蒂皮瓣静脉危象的效果。方法随机选取2017年2月～2018年2月我院收治的50例带蒂皮瓣静脉危象患者作为研究对象,依据低分子肝素钙注射方式将其分为皮瓣淤血区局部真皮下注射+小切口放血组(皮瓣淤血区组,n=25)和脐周皮下注射+皮瓣淤血区小切口放血组(脐周组,n=25),比较两组患者的皮瓣坏死率、活化部分凝血酶时间(APTT)、皮瓣下血肿发生情况。结果皮瓣淤血区组患者的皮瓣坏死率显著低于脐周组,差异有统计学意义(P<0.05);皮瓣淤血区组患者的APTT显著短于脐周组,差异有统计学意义(P<0.05)。两组患者的皮瓣下血肿发生率比较,差异无统计学意义(P>0.05)。结论低分子肝素钙皮瓣淤血区局部真皮下注射+小切口放血治疗带蒂皮瓣静脉危象效果较脐周皮下注射+皮瓣淤血区小切口放血好。

Objective To compare the effect of Low Molecular Weight Heparin Calcium with different injection methods in the treatment of venous crisis of pedicle flaps.Methods A total of 50 patients with venous crisis of pedicle flap in our hospital from February 2017 to February 2018 were randomly selected as the research objects.According to the different injection methods of Low Molecular Weight Heparin Calcium,they were divided into local subdermal injection in flap congestion area plus bloodletting with a small incision group(the flap congestion area group,n=25)and umbilical subcutaneous injection plus bloodletting with a small incision in the flap congestion area group(the umbilical group,n=25).The flap necrosis rate,activated partial thromboplastin time(APTT),and hematoma under the flap were statistically compared between the two groups.Results The flap necrosis rate in the flap congestion area group was significantly lower than that in the umbilical group,and the difference was statistically significant(P<0.05).The APTT in the flap congestion area group was significantly shorter than that in the umbilical group,and the difference was statistically significant(P<0.05).However,there was no significant difference in the incidence of subcutaneous hematoma between the two groups(P>0.05).Conclusion Compared with umbilical subcutaneous injection plus bloodletting with a small incision in the flap congestion area,Low Molecular Weight Heparin Calcium with local subdermal injection in flap congestion area plus bloodletting with a small incision obtains a better effect on treating venous crisis of pedicle flap.
