延续护理在脑卒中出院患者跌倒预防中的应用效果

Application effect of transitional care on preventing falls of stroke patients discharged from hospital

目的探讨延续护理在脑卒中(CVA)出院患者跌倒预防中的应用效果。方法选取2017年1～10月我院收治的CVA患者100例,采用随机数字表法分为对照组和观察组,每组各50例。对照组出院前予以常规出院指导,出院后1周电话随访。观察组除常规出院指导外,建立个人信息档案及观察表,按护理程序给予紧密追踪随访干预。两组均根据功能独立性评定(FIM)量表进行功能恢复评定,并比较两组出院后6个月内跌倒发生率。结果出院6个月,观察组FIM功能独立性评分显著高于对照组,跌倒率显著低于对照组,差异均有统计学意义(P<0.05)。结论延续护理有利于患者日常生活功能恢复,可有效预防CVA患者出院后跌倒发生率。

Objective To investigate the application effect of transitional care on fall prevention in stroke patients discharged from hospital.Methods A total of one hundred stroke patients treated in our hospital from January to October 2017 were selected in this study and they were divided into control group and observation group according to random number table method,50 cases in each group.The control group was given conventional discharge instructions,and followed up by telephone visit after one week.Besides routine discharge guidance,the observation group was established personal record and observation tables,followed up by closely transitional care and intervention.Two groups were assessed for functional recovery by Functional Independence Measure(FIM)table,and the incidence of falls within six months after discharge was compared.Results The score of FIM in the observation group was significantly higher than control group,and the fall incidence was significantly lower than control group,the differences were statistically significant(P<0.05).Conclusion Transitional care is beneficial to the recovery of daily activity of stroke patients,and can effectively prevent the incidence of falls after discharged from hospital.
