早期活动在降低慢性阻塞性肺疾病机械通气病人下肢深静脉血栓形成中的作用

Role of early activities in reducing deep venous thrombosis in patients with mechanical obstruction of chronic obstructive pulmonary disease

[目的]探讨早期活动在预防慢性阻塞性肺疾病(COPD)机械通气病人下肢深静脉血栓形成(DVT)中的作用。[方法]将某三级医院重症医学科收治的126例慢性阻塞性肺疾病病人按随机数字表法分成试验组和对照组各63例,对照组给予慢性阻塞性肺疾病常规护理,试验组在给予慢性阻塞性肺疾病常规护理基础上实施早期活动方案,分别在干预前及干预后第10天测量血浆D-二聚体水平、两组病人股静脉血流速度及观察有无DVT相关症状发生。[结果]试验组血浆D-二聚体水平明显低于对照组,试验组股静脉血流速度明显高于对照组,试验组DVT相关症状发生少于对照组,两组比较差异有统计学意义(P<0.01)。[结论]早期活动方案能降低血浆D-二聚体水平、提高下肢静脉血流速度,安全、有效地降低慢性阻塞性肺疾病机械通气病人下肢DVT形成的风险。

Objective:To investigate the role of early activities in the prevention of deep venous thrombosis(DVT)in patients with mechanical obstruction of chronic obstructive pulmonary disease(COPD)Method:A total of 126 patients with COPD admitted to the intensive care department of a tertiary hospital were divided into the experimental group and the control group according to the random number table method,63 cases in each group.The control group received routine nursing care for COPD.The experimental group implemented the early activity based on the routine nursing of COPD.Plasma D dimer levels,the femoral venous blood flow velocity,and the presence or absence of DVT related symptoms were measured and observed between the two groups before and 10 days after intervention.Results:The plasma D dimer level in the experimental group was significantly lower than that in the control group;the blood flow velocity of the femoral vein in the experimental group was significantly higher than that in the control group;the DVT related symptoms in the experimental group occurred less than the control group,the difference between the two groups was statistically significant(P<0.01).Conclusions:The early activity program could reduce the plasma d dimer level,increase venous blood flow velocity of lower limbs,and safely and effectively reduce the risk of lower extremity DVT in patients with mechanical COPD.
