替罗非班联合经皮冠脉介入治疗急性ST段抬高型心肌梗死的效果分析

Effect of Tirofiban Combined with Percutaneous Coronary Intervention on Acute ST-Segment Elevation Myocardial Infarction

目的探析替罗非班与经皮冠脉介入联合使用治疗急性ST段抬高型心肌梗死(STEMI)的临床效果。方法方便选取该院2015年1月—2017年12月收治的150例急性STEMI患者,随机分为对照组和治疗组,每组75例,对照组采用肝素进行治疗,治疗组在对照组基础上采用替罗非班与经皮冠脉介入联合治疗。结果治疗组患者的TIMI血流Ⅲ级共57例(76.00%),多于对照组的血流分级Ⅲ级患者,差异有统计学意义(χ~2=6.903 7, P<0.05)。而治疗组Ⅱ级患者共8例(10.67%),低于对照组Ⅱ级患者,差异有统计学意义(χ~2=4.649 3, P<0.05)。治疗后治疗组患者的心功能、血浆BNP水平明显优于对照组。3个月后,治疗组的主要心脏不良事件发生率(5.33%)低于对照组,差异有统计学意义(χ~2=5.153, P<0.05)。结论替罗非班与经皮冠脉介入联合使用治疗STEMI患者的综合效果较好,在临床治疗中具有重要的借鉴价值。

Objective To investigate the clinical effects of tirofiban combined with percutaneous coronary intervention in the treatment of acute ST-segment elevation myocardial infarction(STEMI).Methods 150 patients with acute STEMI admitted to the hospital from January 2015 to December 2017 were convenient selected and randomly divided into control group and treatment group,75 cases in each group.The control group was treated with heparin.The treatment group was used on the basis of the control group tirofiban combined with percutaneous coronary intervention.Results A total of 57 patients(76.00%)with TIMI blood flow in the treatment group were more than the blood flow grade III patients in the control group.The difference was statistically significant(χ2=6.903 7,P<0.05).There were 8 patients(10.67%)in the treatment group,which was lower than the control group II patients.The difference was statistically significant(χ2=4.649 3,P<0.05).The heart function and plasma BNP levels of the treatment group were significantly better than those of the control group.After three months,the incidence of major cardiac adverse events in the treatment group(5.33%)was lower than that in the control group,and the difference was statistically significant(χ2=5.153,P<0.05).Conclusion The combination of tirofiban and percutaneous coronary intervention for STEMI patients has a good comprehensive effect and has important reference value in clinical treatment.
