静脉滴注人免疫球蛋白与人血白蛋白冲管液体安全量测定方法分析 被引量:1

摘要 目的探究静脉滴注人免疫球蛋白与人血白蛋白冲管液体安全量测定方法。方法采用滴注与微泵注射2种方式冲管,速度分别为20滴/min、30滴/min、40滴/min、60滴/min与10 ml/h、30 ml/h,对不同时段冲管液吸光度(A)予以紫外分光光度法测定,对浓度下降至相同所需的冲管液体体积进行比较。结果随着冲管液体量增加,冲管液药物浓度呈现下降趋势;冲至管路药物浓度相同时,与其他滴速相比,20滴/min需要量增加,人血白蛋白需要量较静脉滴注人免疫球蛋白多;微泵10 ml冲管、滴注40 ml能够将药液浓度下降至1%以下;当冲管液药物浓度下降到0.1 g/L,人血白蛋白静脉滴注冲管所需液体量为65 ml,静脉滴注人免疫球蛋白所需液体量为55 ml,微泵冲管分别为12 ml、14 ml;当A降至0.1以下,人血白蛋白静脉滴注冲管及静脉滴注人免疫球蛋白所需液体量分别为65 ml、55 ml,此时微泵冲管均为14 ml。结论滴注冲管55 ml、65 ml、微泵冲管14ml能够使静注人免疫球蛋白及人血白蛋白输液管浓度降到安全范围。 Objective To determine the safet yvolume of flushing fluid during intravenous use of human immunoglobulin and human serum albumin.Methods Dripping and micro-pumping were used to flush intravenous lines at speeds of 20 drips/min,30 drips/min,40 drips/min,60 drips/min,10 mL/h and 30 mL/h,respectively.The ultraviolet spectrophotometry was used to determine the absorbance(A)of the flushing fluid at different time points.Volumes of flushing fluid needed to achieve asame reduction in bio-agent concentrations were compared among variousflushing speeds.Results The drug concentration in the flushing fluid showed a decreasing trend along with the increasing volume of flushing fluid.When the concentrations of the drugs in the intravenous lines were the same,greater volume with 20 drips/min wasrequired compared with the other dripping speeds,and this was more obvious during administration of human serum albumin compared with intravenous human immunoglobulin.Flushing with 10 ml fluid by micro-pumping or 40 ml by dripping may reduce the drug concentration in the flushing fluid to less than 1%.To reduce the drug concentration in the flushing fluid to below 0.1g/L,the flushing volume needed by dripping was 65 ml for human serum albumin and 55 ml for human immunoglobulin;these figures were 12 ml and 14 ml,respectively,by micro-pumping.To reduce the A value to below 0.1,the flushing volume needed by dripping was 65 ml for human serum albumin and 55 ml for human immunoglobulin;these figures were both 14 ml by micro-pumping.Conclusion Flushing volumes of 55 ml and 65 ml by dripping and 14 ml by micro-pumping are eligible to reduce the concentrations of intravenous human immunoglobulin and human serum albumin in the intravenous lines to a safety range.
