摘要
(1)目的探讨依达拉奉能否减轻脑膜瘤切除术所引发的炎性反应和脑缺血缺氧反应,衡量依达拉奉的脑保护作用。(2)方法选择我院2017年1~11月,择期行首次脑膜瘤切除术患者30例,随机分为依达拉奉组(观察组)和对照组,每组15例。观察组在麻醉诱导前30分钟内静脉滴注含30mg依达拉奉的生理盐水100mL;对照组仅给予100mL生理盐水,其他用药两组相同。分别于麻醉诱导前(T0)、手术开始(T1)、切开硬脑膜即刻(T2)、术毕(T3)采集颈内静脉血测定肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)、超氧化物歧化酶(SOD)、中枢神经特异蛋白(S100β蛋白)的水平。(3)结果组内比较:与T0相比,两组TNF-α、IL-10、S100β蛋白含量随手术时间的延长,均逐渐升高,SOD含量对照组下降,观察组升高,且各时间点之间比较差异均有统计学意义(P <0.05)。组间比较:两组TNF-α、IL-10含量在T0和T1时间点比较,差异无统计学意义(P>0.05),在T2和T3时点时,对照组的TNF-α高于观察组,IL-10低于观察组,差异有统计学意义(P <0.05)。两组S100β蛋白和SOD含量在T0时间点比较,差异无统计学意义(P>0.05)。在T1、T2和T3时点时,对照组的S100β均高于观察组,SOD均低于观察组,差异有统计学意义(P <0.05)。(4)结论依达拉奉可减轻脑膜瘤切除术围手术期的炎性反应,并且具有对抗自由基,对抗氧化应激反应的作用,对脑膜瘤切除术围手术期具有一定的脑保护作用。
Objective To explore whether edaravone can relieve inflammatory reaction and cerebral ischemia and hypoxia reaction in meningioma patients undergoing meningioma resection,and measure the brain protective effect of edaravone.Methods Thirty patients scheduled for the first resection of meningiomas were selected from January to November 2017 and randomly divided into the edaravone group(observation group)and the control group,and each group had 15 patients.The two groups were treated all the same except the research group was intravenously injected with 100mL saline containing 30mg edaravone before 30min induction,and the control group was only given 100mL normal saline.Collected the internal jugular vein blood during before anesthesia induction(T 0),beginning of operation(T 1),incision of dura mater(T 2),immediately after operation(T 3)and recorded the level of TNF-α,IL-10,superoxide dismutase(SOD)and S100βprotein.Results Within group the expression of TNF-α,IL-10 and S100βprotein in two groups increased gradually with time,but the level of SOD decreased in control group,and increased in observation group,this difference was statistically significant at different time points(P<0.05).The levels of TNF-αand IL-10 were not significantly different between two groups at T 0 and T 1.At T 2 and T 3 the level of TNF-αof control group was higher than that of control group,and the IL-10 level was lower than that of the observation group.There was no significant difference between the two groups of S100βprotein and SOD at T 0(P>0.05).At T 1,T 2 and T 3,S100βin control group was higher than that in observation group,and SOD was lower than that in observation group,and the difference was statistically significant(P<0.05).Conclusion Edaravone can resist the perioperative inflammatory response,free radicals and oxidative stress in meningioma patients,and it has a certain brain protection effect during perioperative period of meningioma resection.
