温郁金免煎颗粒联合三联疗法根除幽门螺杆菌疗效观察

Clinical observation of helicobacter pylori with YujinYijian granules combined with triple therapy

(1)目的比较在标准三联疗法的基础上辅以温郁金免煎颗粒与包括铋剂的四联疗法对幽门螺杆菌(H.pylori,HP)的根除疗效、临床症状改善情况以及药物副作用。(2)方法选择14 C-尿素呼气实验(14C-UBT)确诊幽门螺杆菌阳性患者120例,按照随机法将其分为两组。A组给予三联疗法(雷贝拉唑钠肠溶胶囊+阿莫西林胶囊+呋喃唑酮片)+温郁金免煎颗粒治疗;B组(对照组)给予三联疗法+枸橼酸铋钾胶囊治疗,两组用药均为10天。采用填写信息登记表、电话回访及门诊随访的方式,分别于治疗前后对患者的临床症状进行评分,通过治疗前后的临床症状积分变化评价治疗效果,比较治疗过程中出现的药物不良反应发生率,并对停药4周后的患者复查14 C-UBT。(3)结果 A组HP根除率高于B组,但差异无统计学意义(P>0.05)。A组在临床疗效、临床症状改善及不良反应发生率方面显著高于B组,差异有统计学意义(P <0.05)。(4)结论两组治疗方案在根除HP方面均有效,且效果相当;温郁金免煎颗粒联合标准三联疗法在临床疗效、临床症状改善及降低不良反应方面均优于含铋剂的四联疗法,可替代枸橼酸铋钾组成新"四联疗法"在临床推广。

Objective To compare the effect of the triple therapy combined with Yujin Yijian granule on the eradication curative effect of H.pylori(HP)eradication and improvement of clinical symptoms and side effects.Methods A total of 120 cases of Helicobacter pylori-positive patients diagnosed by 14 C-urea breath test(14 C-UBT)were randomly divided into two groups.Group A was given triple therapy(ie,rabeprazole sodium enteric-coated capsules+amoxicillin capsules+furazolidone tablets)+Yujin Yijian granules.Group B(control group)was given triple therapy(ie,rabeprazole)sodium enteric-coated capsule+amoxicillin capsule+furazolidone Tablet)+Potassium Citrate Potassium Capsule Treatment,and both groups were treated for 10 days.Patients'clinical symptoms were scored before and after treatment by filling in information registration forms,telephone interviews and outpatient follow-ups.Treatment effects were evaluated by the changes in clinical symptom scores before and after treatment,and drug abuse during treatment was compared.Differences in response rates and review of 14 C-UBT in patients after 4 weeks of discontinuation.Results The eradication rate of H.pylori in group A was higher than that in group B,and the difference was not statistically significant(P>0.05).The clinical efficacy,improvement of clinical symptoms and incidence of adverse reactions in group A were significantly higher than those in group B.The difference was statistically significant(P<0.05).Conclusion Two groups of drug treatment programs are effective in eradicating H.pylori and the efficacy is comparable.The combination of YujinYijian granules and standard triple therapy is superior to the quadruple therapy containing tincture in terms of clinical efficacy,improvement of clinical symptoms,and reduction of adverse reactions.It has significant effects and can be used as a substitute for bismuth potassium citrate to form a new"quadruple combination"."Therapeutic"is worth promoting in the clinical treatment of eradication of Helicobacter pylori.
