甘肃省23个特困县(区)卫生资源配置效率研究

Study on the efficiency of health resource allocation in 23 poor counties(districts) of Gansu Province

[目的]对甘肃省23个特困县(区)卫生资源配置效率进行研究,为甘肃省提高卫生资源的利用效率、实现资源的优化配置提供科学依据。[方法]运用数据包络分析模型(DEA),选用医师数、注册护士数、床位数、机构数为投入指标,年诊疗人次、年住院人数、病床使用率为产出指标,从横向和纵向2个维度对甘肃省23个特困县(区)卫生资源的利用效率进行研究。[结果]甘肃省23个特困县(区)卫生资源配置效率存在地域差异; 5个特困县(区)卫生资源配置效率相对较低,表现出非DEA有效; 2013-2017年23个特困县(区)卫生资源配置效率总体不断优化; 2015-2017年3年规模效率不变。[结论]甘肃省23个特困县(区)卫生资源总体配置情况不甚合理。影响和制约甘肃省23个特困县(区)卫生资源配置的主要问题包括配置体系、配置结构、管理制度等方面。

Objective To study the efficiency of health resources allocation in 23 poverty-stricken counties(districts)in Gansu Province so as to provide scientific basis for improving the utilization efficiency of health resources and optimizing the allocation of resources in Gansu Province.Methods It used data envelopment analysis model(DEA),selected the number of physician,registered nurse,bed and institution as input index,the number of annual diagnosis and treatment person-time,the number of annual hospitalization and utilization rate of hospital beds as output index,studied the efficiency of health resources allocation in 23 poverty-stricken counties(districts)in Gansu Province from crosswise and lengthways two dimension.Results There were geographical differences in the efficiency of health resources allocation in 23 counties(districts),five of them were relatively low,showed up non-DEA efficiency.From 2013 to 2017,the total of efficiency of health resources location in 23 poverty-stricken counties(districts)had been optimizing,and the scale efficiency had been unchanged for three years from 2015 to 2017.Conclusions The overall allocation situation of health resources allocation in 23 poverty-stricken counties(districts)in Gansu Province is unreasonable.Main problems of health resources allocation include location system,location structure and management system.
