摘要
目的探讨超微血管成像技术对浅表淋巴结疾病的诊断价值。方法回顾分析84例患者(102枚淋巴结)的超声检查资料。以病理结果为金标准,比较二维灰阶超声分别联合彩色多普勒、能量多普勒、超微血管成像及超声造影鉴别诊断良恶性淋巴结的准确率以及诊断恶性淋巴结的敏感性、特异性,并比较4组间准确率有无统计学意义。结果二维灰阶超声分别联合超微血管成像及超声造影诊断病变淋巴结准确率均高于二维灰阶超声分别联合彩色多普勒及能量多普勒,差异均有统计学意义,而二维灰阶超声分别联合超微血管成像及超声造影诊断准确率差异无统计学意义。二维灰阶超声联合超微血管成像及超声造影诊断恶性淋巴结的敏感性、特异性均高于二维灰阶超声联合彩色多普勒及能量多普勒。结论超微血管成像与超声造影较彩色多普勒与能量多普勒有更高的微血管显示率,超微血管成像技术有助于提高浅表病变淋巴结诊断的准确率。
Objective To investigate the diagnostic value of superb microvascular imaging for superficial lymph node disease.Method The ultrasonographic data of 84 patients(102 lymph nodes)were retrospectively analyzed.With the pathological results as the gold standard,to compare the accuracy of differential diagnosis of benign and malignant lymph nodes with two-dimensional gray-scale ultrasound combined with color Doppler,power Doppler,superb microvascular imaging and contrast-enhanced ultrasound,and to diagnose the sensitivity and specificity of malignant lymph nodes,And compare the accuracy of the four groups was statistically significant.Results The diagnostic accuracy of two-dimensional gray-scale ultrasound combined with superb microvascular imaging and contrast-enhanced ultrasonography was higher than that of two-dimensional gray-scale ultrasound combined with color Doppler and power Doppler respectively,and the differences were statistically significant.Two-dimensional gray-scale ultrasound combined with superb microvascular imaging and contrast-enhanced ultrasound diagnostic accuracy was not statistically significant.The sensitivity and specificity of two-dimensional gray-scale ultrasound combined with superb microvascular imaging and contrast-enhanced ultrasound in the diagnosis of malignant lymph nodes are higher than those of two-dimensional gray-scale ultrasound combined with color Doppler and power Doppler.Conclusions Superb microvascular imaging and contrast-enhanced ultrasound have higher microvascular display rate than color doppler and power doppler.Superb microvascular imaging helps to improve the accuracy of the qualitative diagnosis of superficial enlarged lymph nodes.
