紫杉醇和顺铂腹腔热灌注化疗治疗卵巢癌腹腔积液的疗效与安全性研究

The Efficacy and Safety Studies of Hyperthermic Intraperitoneal Chemotherapy with Paclitaxel and Cisplatin in Ovarian Cancer Patients with Peritoneal Effusion

目的探讨紫杉醇和顺铂腹腔热灌注化疗治疗卵巢癌腹腔积液的疗效与安全性。方法选取42例采用紫杉醇和顺铂腹腔热灌注化疗以及同期采用同一化疗方案并应用腹腔穿刺的38例卵巢癌腹腔积液患者作为研究对象,分为观察组和对照组。对2组患者治疗后的近期疗效、引流情况以及不良反应进行比较分析。结果观察组有效率较对照组高(P <0. 05)。治疗后2组引流流速均较同组治疗前明显降低(P <0. 05),但观察组相比对照组明显降低(P <0. 05)。2组患者治疗过程中均出现Ⅰ~Ⅱ度骨髓抑制、恶心呕吐、疲乏和发热,无Ⅲ~Ⅳ度不良反应发生,观察组另有Ⅰ度腹泻、血压升高以及蛋白尿各1例,但组间差异无统计学意义(P> 0. 05)。结论紫杉醇和顺铂腹腔热灌注化疗在治疗卵巢癌腹腔积液方面相比传统的灌注引流的近期治疗效果更佳。

Objective To study the efficacy and safety of hyperthermic intraperitoneal chemotherapy with paclitaxel and cisplatin in ovarian cancer patients with peritoneal effusion.Methods To select 42 cases of patients with hyperthermic intraperitoneal chemotherapy by paclitaxel and cisplatin and 38 cases of ovarian cancer patients with peritoneal effusion used the same chemotherapy regimens and with abdominal puncture treatment at the same time as the research objects,and divided into the observation group and the control group respectively,then analyzed and compared the recent efficacy,drainage condition and adverse reactions of patients in the 2 groups.Results The effective rate of the observation group was decreased significantly than that of the control group,and the number of total effective patients was obviously increased;The drainage velocity after treatment of two groups was significantly lower than that of the same group before treatment(P<0.05),but the drainage velocity after treatment of the observation group was significantly decreased than that of the control group(P<0.05);There wereⅠ~Ⅱdegrees of bone marrow suppression,fatigue,nausea and vomiting and fever occurred in two groups in the process of treatment,and noⅢ~Ⅳdegrees of adverse reactions occurred,and the observation group had 1 case each ofⅠdegrees of diarrhea,blood pressure rising and proteinuria,the difference between the 2 groups was no statistically significant(P>0.05).Conclusion The recent treatment effect of hyperthermic intraperitoneal chemotherapy with paclitaxel and cisplatin in ovarian cancer patients with peritoneal effusion is better.
