瘤大块切除术与瘤体刮除术治疗膝关节周围骨巨细胞瘤的疗效和安全性对比

Comparison of the Efficacy and Safety of Lump Lump Resection and Tumor Curettage in the Treatment of Giant Cell Tumor of the Knee Joint

目的对比分析瘤大块切除术与瘤体刮除术治疗膝关节周围骨巨细胞瘤的疗效和安全性。方法选择51例膝关节周围骨巨细胞瘤患者,随机分为2组。对照组采取瘤大块切除术治疗,观察组采取瘤体刮除术治疗,比较2组的临床治疗效果。结果观察组术后的关节功能优良率为88. 00%(22/25),明显高于对照组的65. 38%(17/26)(P<0. 05);观察组术后的肢体功能优良率为92. 00%(23/25),明显高于对照组的73. 08%(19/26)(P <0. 05)。观察组术后局部肿瘤的复发率为24. 00%(6/25),高于对照组的19. 23%(5/26),但差异无统计学意义(P> 0. 05)。观察组术后并发症发生率为4. 00%(1/25),明显低于对照组的11. 54%(3/26)(P <0. 05)。结论与瘤大块切除术相比,瘤体刮除术治疗膝关节周围骨巨细胞瘤具有较好的疗效和安全性,可以显著改善患者的关节功能和肢体功能,可作为首选的手术方式。

Objective To investigate the clinical effect and safety of lump lump resection and tumor curettage in the treatment of giant cell tumor of the knee joint.Methods Selected 51 cases of patients with giant cell tumor of the knee joint,divided into 2 groups randomly.The control group was treated with large lump of tumor,and the observation group was treated with curettage,and the clinical effect of the two groups was compared.Results After treatment,the excellent rate of postoperative joint function of the observation group was 88.00%(22/25),significantly higher than that of the control group 65.38%(17/26)(P<0.05);the excellent rate of postoperative limb function of the observation group was 92.00%(23/25),significantly higher than that of the control group 73.08%(19/26)(P<0.05);The recurrence rate of local tumor in the observation group was 24.00%(6/25),which was higher than that of the control group 19.23%(5/26),but there was no significant difference(P>0.05);In control group,there were 1 cases of graft reaction,2 cases of bacterial infection,1 cases of bacterial infection in the observation group,and 4.00%(1/25)in the observation group,which was significantly lower than that of the control group 11.54%(3/26)(P<0.05).Conclusions Compared with tumor resection,tumor curettage has good efficacy and safety for patients with giant cell tumor around knee joint.It can significantly improve the joint function and limb function of patients,and it can be used as the first choice of operation.
