慢性阻塞性肺疾病患者血清YKL-40与炎症因子、肺功能及预后相关性临床观察

Correlations of serum YKL-40 with inflammatory factors,pulmonary function and prognosis in COPD patients

目的探讨慢性阻塞性肺疾病(COPD)患者YKL-40与肺功能及预后相关性。方法以AECOPD(B组)、COPD(A组)及无肺部疾病且肺功能正常(C组)各60例为研究对象。收集并分析3组人群炎症指标、CAT评分、肺功能及YKL-40差异,探讨YKL-40在不同人群中与CAT评分及肺功能相关性。结果与A及C组比,B组YKL-40、PCT、hs-CRP及IL-6明显升高,而肺功能及CAT评分明显降低,差异均有统计学意义(P <0.05);与C组比,A组YKL-40、PCT、hs-CRP及IL-6明显升高,差异有统计学意义(P <0.05);对AECOPD及COPD患者,YKL-40与PCT、IL-6、hs-CRP及CAT评分呈正相关,与FEV1%呈负相关。结论 YKL-40预测COPD炎症反应程度及评估疾病预后具有一定临床价值。

Objective To investigate the correlations of YKL-40 with pulmonary function and prognosis in patients with chronic obstructive pulmonary disease(COPD).Methods Sixty cases of AECOPD(group B),COPD(group A)and no lung disease and normal lung function(group C)were selected as the subjects.The inflammatory markers,CAT score,lung function and YKL-40 in the 3 groups were analyzed,and the correlations of YKL-40 with CAT score and lung function was investigated.Results Compared with group A and C,YKL-40,PCT,hs-CRP and IL-6 in group B were significantly higher and lung function and CAT score were significantly decreased(P<0.05).Compared with group C,YKL-40,PCT,C and IL-6 in group A were significantly higher(P<0.05).In AECOPD and COPD patients,YKL-40 was positively correlated with PCT,IL-6,hs-CRP and CAT scores,and negatively correlated with FEV1%.Conclusion YKL-40 is clinically of value in predicting the extent of COPD inflammation and evaluating the prognosis of the disease.
