桂枝茯苓丸联合达那唑和孕三烯酮胶囊治疗子宫内膜异位症疗效观察

Therapeutic effects of Guizhifuling pill combined with danazol and gestrinone capsules on endometriosis

目的观察桂枝茯苓丸联合达那唑、孕三烯酮胶囊治疗子宫内膜异位症的临床疗效。方法选取2016年1月至2018年1月行腹腔镜手术的子宫内膜异位症患者110例,随机分为对照组和治疗组,每组55例。对照组术后给予达那唑、孕三烯酮胶囊口服,治疗组在对照组基础上给予桂枝茯苓丸口服。疗程均为12周。观察2组临床疗效、症状改善程度、性激素[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)]、CA125、CA199水平、不良反应、妊娠结局和复发情况。结果治疗组总有效率显著高于对照组,差异有统计学意义(P <0. 05)。与治疗前比较,2组治疗后囊肿体积、子宫活动度、子宫内膜厚度、痛经评分、FSH、LH、E2、P、CA125、CA199水平均显著低于治疗前,且治疗组改善程度优于对照组(P <0. 05)。治疗组不良反应发生率、复发率显著低于对照组,治疗后2年内妊娠率、分娩率显著高于对照组,差异有统计学意义(P <0. 05)。结论桂枝茯苓丸联合达那唑、孕三烯酮胶囊治疗子宫内膜异位症疗效确切,能够有效缓解痛经症状,降低性激素水平,提高妊娠率,值得临床推广应用。

Objective To observe the therapeutic effects of Guizhifuling pill combined with danazol and gestrinone capsules on endometriosis.Methods A total of 110 patients with endometriosis who were treated by laparoscopic operation in our hospital from January 2016 to January 2018 were randomly divided into control group(n=55)and treatment group(n=55).The patients in control group were treated by oral danazol and gestrinone capsules after operation,however,the patients in treatment group,on the basis of control group,were treated by Guizhifuling pill,with a treatment course of 12 weeks for both groups.After treatment,the clinical curative effects,symptoms improvement degree,the levels of FSH,LH,E2,P,CA125 and CA199,and the adverse reactions,the recurrence rate and pregnancy outcome were observed and compared between the two grousp.Results The total effective rate in treatment group was significantly higher than that in control group(P<0.05).As compared with those before treatment,endometrial cysts size,activity of uterus,endometrial thickness,the pain scores,the levels of FSH,LH,E2,P,CA125 and CA199 were significantly decreased after treatment in both groups,moreover,the above indexes in treatment group were superior to those in control group(P<0.05).The incidence of adverse reactions and relapse rate in treatment group were significantly lower than those in control group,however,the pregnancy rate and childbirth rate within 2 years after treatment in treatment group were significantly higher than those in control group(P<0.05).Conclusion Guizhifuling pill combined with danazol and gestrinone capsules has definite therapeutic effects on endometriosis,which can significantly relieve symptoms of dysmenorrhea,reduce sex hormone levels and improve the pregnancy rate,therefor,which is worthy of application and promotion in clinical practice.
