纯钛桩核和石英纤维桩核联合铸瓷全冠在修复磨牙缺损中的疗效观察 被引量:8

摘要 目的观察纯钛桩核和石英纤维桩核联合铸瓷全冠在修复磨牙缺损中的疗效。方法选取2014—2015年在郑州大学第一附属医院口腔医学中心就诊的86例(117颗牙)残冠患者,按照治疗方法分为3组,A组接受石英纤维桩+树脂核治疗,B组接受铸造纯钛桩核治疗,C组为空白对照组,最后运用CAD/CAM全瓷修复系统制作全瓷冠。随访观察3 a,评价临床效果。结果 A组4颗患牙的纤维桩核发生脱落,1颗患牙出现牙龈炎症,1颗桩折,修复成功率为85. 4%(35/41); B组发生1颗牙龈炎症,3颗根折,修复成功率为86. 8%(33/38); C组出现1颗继发龋,12颗根折,修复成功率为64. 9%(24/37)。A组和B组修复成功率比较,差异无统计学意义(P> 0. 05),A组、B组与C组修复成功率比较,差异有统计学意义(P <0. 05)。A组桩折的发生率与B组比较,差异无统计学意义(P> 0. 05)。A组桩脱位的发生率高于B组,差异有统计学意义(P <0. 05)。C组根折的发生率高于A、B组,B组高于A组,差异有统计学意义(P <0. 05)。结论纤维桩核的根折率低于金属桩核,但纤维桩核的脱落发生率高,无桩修复的根折率大于纤维桩核和金属桩核。 Objective To observe the effect of pure titanium post core and quartz fiber post core combined with cast porcelain crown in repairing molar defect.Methods Eighty-six(117 teeth)patients with residual crown who visited the First Affiliated Hospital Stomatology Center of Zhengzhou University from 2014 to 2015 were selected,who were divided into three groups according to treatment methods.Group A accepted quartz fiber post and resin core treatment.Group B accepted cast pure titanium post core treatment.Group C was a blank control group.The CAD/CAM all-ceramic repair system was used to make the all-ceramic crown finally.After three years of follow-up observation,the clinical effect was evaluated.Results The fiber post and core of four suffering teeth in group A fell off.One suffering tooth occured gingival inflammation.One pile fold.The success rate of repair was 85.4%(35/41).One gingival inflammation occurred in group B.Three root folds.The success rate of repair was 86.8%(33/38).Group C occured one secondary sputum.Twelve root folds.The success rate of repair was 64.9%(24/37).Repair success rates between group A and group B were compared,and the difference was not statistically significant(P>0.05).Repair success rates among group A,B and C were compared,and the difference was statistically significant(P<0.05).The incidence of pile folding in group A was compared with that of group B,and the difference was not statistically significant(P>0.05).The incidence of dislocation of the pile in group A was higher than that of group B,and the difference was statistically significant(P<0.05).The incidence of root fracture in group C was higher than that of group A and group B,and the incidence of root fracture in group B was higher than that of group A,and the difference was statistically significant(P<0.05).Conclusion The root fracture rate of the fiber post core is lower than that of the metal post core,however,the incidence of shedding of the fiber post core is high.The root-break rate of pile-free repair is greater than that of fiber post core and metal post core.
