生存素和同型半胱氨酸水平对PCI术后支架内再狭窄的影响及二者间相关性的研究

摘要 目的:研究生存素(survivin,SVV)和同型半胱氨酸(homocysteine,HCY)水平对经皮冠状动脉介入治疗(per-cutaneous coronary intervention,PCI)术后支架内再狭窄的影响及二者相关性。方法:选取2015年1月至2017年10月间于包头医学院第一附属医院心内科住院并且植入支架,PCI术后规律口服双联抗血小板、调脂、稳定斑块药物至少1年的患者86例,分为支架内狭窄组(30例)和支架内未再狭窄组(56例),分别测定两组患者血中的生存素(SVV)、同型半胱氨酸(HCY)、血糖、血脂水平。结果:两组患者年龄、性别、高血压患病率、糖尿病患病率、吸烟比例、胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白水平等方面的比较差异无统计学意义(P> 0. 05)。支架内再狭窄组与支架内未再狭窄组中SVV浓度接近,两组间无统计学差异(P> 0. 05); SVV浓度与支架内再狭窄间相关性无统计学意义(R=-0. 259,P> 0. 05)。再狭窄组中HCY浓度高于未再狭窄组(P <0. 05),差异具有统计学意义; HCY浓度与支架内再狭窄呈正相关(R=0. 779,P <0. 001)。SVV与HCY间无明显相关性(P> 0. 05)。结论:(1)本实验中生存素(SVV)与支架内再狭窄间的相关性无统计学意义,提示生存素可能并未起到促进支架再狭窄的作用;(2)同型半胱氨酸与支架内再狭窄间呈正相关,提示同型半胱氨酸是支架内再狭窄的危险因素;(3)生存素(SVV)与同型半胱氨酸(HCY)间无明显相关性。 Objective:To investigate the effect of survivin(SVV)and homocysteine(HCY)expression on in-stent restenosis after percutaneous coronary intervention(PCI)and their correlation.Methods:80 samples were chosen from patients who were hospitalized in the Department of Cardiology of the First Affiliated Hospital of Baotou Medical College from January 2015 and October 2017.Oral anti-platelet,lipid-regulating,and plaque-stabilizing drugs were administered orally in these patients for at least 1 year after PCI.The patients were divided into two groups:30 patients in the in-stent restenosis group and 56 patients in the in-stent non-restenosis group.The levels of SVV,HCY,blood sugar and blood lipids in the two groups were estimated.Results:There was no significant difference in age,gender,prevalence rate of hypertension,diabetes prevalence,smoking rate,and the levels of cholesterol,triglycerides,low-density lipoprotein and high-density lipoprotein between the two groups(P>0.05).The concentration of SVV in the in-stent restenosis group was close to that in the in-stent non-restenosis group,and there was no statistical difference between the two groups(P>0.05).The correlation between SVV concentration and restenosis was not statistically significant(R=-0.259,P>0.05).HCY concentration in the in-stent restenosis group was higher than that in the in-stent non-restenosis group(P<0.05),and the difference was statistically significant.HCY concentration was positively correlated with in-stent restenosis(R=0.779,P<0.001).There was no significant correlation between SVV and HCY(P>0.05).Conclusion:(1)The correlation between SVV and in-stent restenosis was not statistically significant in this experiment,suggesting that SVV may not play a role in promoting stent restenosis;(2)There is a positive correlation between HCY and in-stent restenosis,suggesting that HCY is a risk factor for in-stent restenosis;(3)There is no significant correlation between SVV and HCY.
