摘要
目的:探讨空腹血糖与心肌梗死后左室射血功能障碍及左室结构改变的相关性。方法:768例心肌梗死患者根据空腹血糖四分位数进行分组,采用广义线性模型比较各组间左室结构参数和射血分数的均数差值,以空腹血糖四分位数的均值作为连续性变量,计算左室射血分数降低和结构改变的线性趋势。结果:在校正年龄、性别、生活方式、血脂水平和肾功能等各种危险因素后,空腹血糖四分位数最高组与四分位数最低组相比,左室射血分数降低1.66%(95%CI:-3.06~-0.26,P_(trend)=0.02),左室舒张末期内径增加1.14 mm(95%CI:0.07~2.22,P_(trend)=0.04),左室收缩末期内径增加1.21 mm(95%CI:0.13~2.30,P_(trend)=0.03)。结论:空腹血糖升高可导致心肌梗死后左室收缩功能不良。
Objective:To explore the correlation between fasting plasma glucose(FPG)and left ventricular ejection dysfunction and left ventricular structure changes after myocardial infarction.Methods:A total of 768 patients with myocardial infarction were divided into groups according to FPG quartile.We compared left ventricular structural parameters and ejection fraction among different groups by generalized linear model,and the linear trend were calculated.Results:After the adjustment of age,gender,lifestyle,serum lipid and serum creatinine,the average left ventricular ejection fraction was 1.66%lower(95%CI:-3.06^-0.26,P trend=0.02),with an increase of 1.14 mm(95%CI:0.07~2.22,P trend=0.04)in left ventricular end-diastolic diameter and 1.21 mm(95%CI:0.13~2.30,P trend=0.03)in left ventricular systolic dimension in the lowest quartile group than those in the highest quartile group.Conclusions:Increase of FPG may lead to poor left ventricular systolic function after myocardial infarction.
