老年高血压患者血清尿酸水平与代谢综合征的相关性研究

Relationship between serum uric acid level and metabolic syndrome in elderly hypertensive patients

目的探讨老年高血压患者血清尿酸水平与代谢综合征(MS)及其各组分的相关性。方法选取老年原发性高血压患者431例,根据是否合并MS分为MS组204例和非MS组227例。又根据患者罹患MS组分数目,分为1组102例、2组125例、3组145例、4+5组59例。分析血清尿酸水平与MS及其各组分的关系。应用ROC曲线分析血清尿酸的截断点。结果与非MS组比较,MS组糖尿病、收缩压、体质量指数、腰围、腰臀比、空腹血糖、TG及尿酸水平明显升高,HDL-C水平明显降低,差异有统计学意义(P<0.01)。1组、2组、3组、4+5组空腹血糖、TG、HDL-C和尿酸水平比较,差异有统计学意义(P<0.01)。二分类logistic回归分析显示,尿酸为MS的危险因素(OR=1.005,95%CI:1.001~1.009,P=0.020)。Pearson相关性分析显示,血清尿酸水平与腰围、TG呈正相关,与HDL-C呈负相关(P<0.01)。ROC曲线分析得出截断值为339.25,敏感性为47.5%,特异性为73.6%(ROC曲线下面积=0.604,95%CI:0.550~0.657,P=0.000)。结论老年原发性高血压患者血清尿酸水平与MS及其各组分具有相关性,建议老年高血压患者应该更为严格地控制尿酸水平。

Objective To study the relationship of serum UA level with MS and its components in elderly hypertensive patients.Methods Four hundred and thirty-one elderly hypertensive patients admitted to our hospital were divided into MS group(n=204)and MS-free group(n=227).The patients were further divided into group 1(n=102),group 2(n=125),group 3(n=145),and group 4+5(n=59)according to their MS components.The relationship between serum UA level and MS and MS components was analyzed.The cut-off value of serum UA was analyzed according to its ROC curve.Results The incidence of DM,SBP and BMI,serum FPG,TG,UA levels and waist/hip ratio were significantly higher,the waist circumference was significantlt longer while the serum HDL-C level was significantly lower in MS group than in MS-free group(P<0.01).The serum FPG,TG,HDL-C and UA levels were significantly different in groups 1,2,3 and 4+5(P<0.01).Logistic regression analysis showed that UA was a risk factor for MS(OR=1.005,95%CI:1.001-1.009,P=0.020).Pearson correlation analysis revealed that serum UA level was positively related with waist circumference and serum TG level and negatively related with serum HDL-C level(P<0.01).ROC curve analysis displayed that the cut-off value of serum UA was339.25 with a sensitivity of 47.5% and a specificity of 73.6%(AUC=0.604,95%CI:0.550-0.657,P=0.000).Conclusion The serum UA level is related with MS and its components in elderly hypertensive patients and should thus be strictly controlled.
