达菲林和米非司酮治疗腹腔镜术后子宫内膜异位症的效果比较

Comparison on the Effects of Diphereline and Mifepristone in the Treatment of Endometriosis after Laparoscopic Surgery

目的比较达菲林和米非司酮治疗腹腔镜术后子宫内膜异位症的效果。方法选取我院2013年3月至2016年3月行腹腔镜手术的子宫内膜异位症患者80例,随机分为两组各40例。观察组给予达菲林治疗,对照组给予米非司酮治疗。观察两组患者的治疗效果、血清雌二醇水平、痛经和不良反应情况。随访2年,比较两组患者的子宫内膜异位症复发率。结果观察组的总有效率明显高于对照组(P <0.05),而复发率明显低于对照组(P <0.05)。治疗后,两组患者的血清雌二醇水平、痛经评分均显著降低,且观察组的血清雌二醇水平、痛经评分明显低于对照组(P均<0.05)。观察组的不良反应发生率明显高于对照组(P <0.05)。两组的肝功能异常发生率比较差异无统计学意义(P>0.05)。结论达菲林治疗腹腔镜术后子宫内膜异位症的疗效更佳,复发率较低,但不良反应发生率较米非司酮更高,临床治疗时应综合考虑使用。

Objective To compare the effects of diphereline and mifepristone in the treatment of endometriosis after laparoscopic surgery.Methods 80 cases of patients with endometriosis undergoing laparoscopic surgery in our hospital from March 2013 to March 2016 were selected and randomly divided into two groups,with 40 cases in each group.The observation group was treated with diphereline,while the control group was treated with mifepristone.The treatment effect,serum estradiol level,dysmenorrhea and adverse reactions of the two groups were observed.After 2 years of follow up,the recurrence rate of endometriosis was compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05),and the recurrence rate was significantly lower than that of the control group(P<0.05).After treatment,the serum estradiol levels and dysmenorrhea scores of two groups significantly decreased,and the serum estradiol level and dysmenorrhea score of the observation group were significantly lower than those of the control group(all P<0.05).The incidence of adverse reactions of the observation group was significantly higher than that of the control group(P<0.05).No statistical difference was found in the incidence of liver function abnormality between the two groups(P>0.05).Conclusions Diphereline in the treatment of endometriosis after laparoscopic surgery has better curative effect and lower recurrence rate,but has higher incidence of adverse reactions compared with mifepristone.Clinical treatment should be considered comprehensively.
