覆膜食管支架术的应用及临床病例分析

Application of Covered Esophageal Stenting and Analysis on Clinical Cases

目的探讨覆膜食管支架术的应用效果及分析临床病例。方法选择我院2014年1月至2016年1月行覆膜食管支架术治疗的患者40例,分为两组各20例。对照组采用传统支架治疗,观察组采用镍钛合金覆膜支架治疗。比较两组患者的治疗效果。结果观察组的覆膜食管支架术支架置入成功率高于对照组(P <0.05)。观察组的支架置入时间短于对照组,置入支架时的扩张程度低于对照组(P均<0.05)。两组患者的术后住院时间比较无统计学差异(P>0.05)。术前,两组患者的生活质量评分比较无统计学差异(P>0.05);术后,两组患者的生活质量评分优于术前(P <0.05),且观察组的生活质量评分优于对照组(P <0.05)。观察组的支架移位发生率低于对照组(P <0.05)。两组患者的并发症发生率比较无统计学差异(P>0.05)。结论针织型、传统支架覆膜食管支架术各有优势,可根据患者的情况选择合适的方案。

Objective To study the application effect and analyze the clinical cases of covered esophageal stenting.Methods 40 cases of patients with covered esophageal stenting in our hospital from January 2014 to January 2016 were selected and divided into two groups with 20 cases in each group.The control group received traditional stenting,while the observation group was treated with nitinol covered stent.The treatment effect was compared between the two groups.Results The success rate of covered esophageal stenting of the observation group was higher than that of the control group(P<0.05).The stent placement time of the observation group was shorter than that of the control group,and the expansion degree of stent placement was lower than that of the control group(both P<0.05).No statistical difference was found in the postoperative hospitalization time between the two groups(P>0.05).Before operation,no statistical difference was found in the life quality scores between the two groups(P>0.05).After operation,the life quality scores of the two groups were better than those before operation(P<0.05),and the life quality score of the observation group was better than that of the control group(P<0.05).The incidence of stent displacement of the observation group was higher than that of the control group(P<0.05).No statistical difference was found in incidence of complications between the two groups(P>0.05).Conclusions Knitted type and traditional stent of covered esophageal stenting have their own advantages.The appropriate plan can be chosen according to patients'condition.
