青光眼小梁切除术后患者浅前房的处理与分析

The Treatment and Analysis of Shallow Anterior Chamber after Glaucoma Trabeculectomy

目的探讨青光眼小梁切除术后患者浅前房形成的原因和治疗措施。方法对121例(156眼)青光眼小梁切除术后患者发生浅前房的原因进行分析。结果浅前房28眼,发生率17.95%;其中滤过过畅23眼(82.14%),结膜瓣渗漏3眼(10.71%),脉络膜脱离1眼(3.57%),恶性青光眼1眼(3.57%)。浅前房大多数为滤过过畅,通过非手术治疗前房恢复正常;1眼恶性青光眼患者先采取保守治疗效果欠佳,后经手术治疗前房形成,眼压下降到正常范围。结论针对浅前房发生原因,术前、术中、术后采取相应的预防措施,可减少其发生率。

Objective To introduce the reasons and treatment measures of shallow anterior chamber after glaucoma trabeculectomy.Methods To analyze the reasons of shallow anterior chamber after glaucoma trabeculectomy in the 121 patients(156 eyes).Results Shallow anterior chamber occurred in 28 eyes(17.95%);It was caused by excessive outflow in 23 eyes(82.14%),conjunctival flap leakage in 3 eyes(10.71%),choroidal detachment in 1 eye(3.57%),malignant glaucoma in 1 eye(3.57%).Shallow anterior chamber was mainly caused by excessive outflow,and it returned to normal with non-surgical treatment of anterior chamber;it was ineffective to take conservative treatment in a patient(1 eye)with malignant glaucoma,then after surgery,the anterior chamber formed,and intraocular pressure(IOP)decreased to the normal range.Conclusion Shallow anterior chamber is a common complication after glaucoma trabeculectomy.According to the reasons for occurrence of shallow anterior chamber,taking appropriate preventive measures before,during and after the operation can reduce its incidence.
