不同手术方式对胃十二指肠溃疡急性穿孔患者的治疗效果分析

Analysis on effect of different surgical methods on patients with acute perforation of gastroduodenal ulcer

目的分析不同手术方式对胃十二指肠溃疡急性穿孔患者的治疗效果。方法 80例胃十二指肠溃疡急性穿孔患者,按照手术方式的不同分为腹腔镜组及开腹组,各40例。腹腔镜组行腹腔镜穿孔修补术,开腹组行开腹穿孔修补术。比较两组患者的手术时间、切口长度、并发症发生情况、术后肠道功能恢复时间。结果腹腔镜组手术时间为(62.33±8.66)min、切口长度为(3.48±0.11)cm、肠道功能恢复时间为(69.76±23.58)h,均优于开腹组的(84.25±12.17)min、(16.12±3.11)cm、(85.36±22.36)h,差异均有统计学意义(P<0.05);两组患者术后并发症发生率均为5.0%,比较差异无统计学意义(P>0.05)。结论对胃十二指肠溃疡急性穿孔患者采取腹腔镜治疗,可以缩短手术时间、减小切口长度,减少并发症的发生,值得在临床推广。

Objective To analyze the effect of different surgical methods on patients with acute perforation of gastroduodenal ulcer.Methods A total of 80 patients with acute perforation of gastroduodenal ulcer were divided by different surgical methods into laparoscopic group and open group,with 40 cases in each group.Laparoscopic group received laparoscopic perforation repair,and the open group received open abdominal perforation repair.Comparison were made on operation time,incision length,incidence of complications and postoperative intestinal function recovery time between the two groups.Results Laparoscopic group had better operation time as(62.33±8.66)min,incision length as(3.48±0.11)cm and intestinal function recovery time as(69.76±23.58)h than(84.25±12.17)min,(16.12±3.11)cm and(85.36±22.36)h in open group,and their difference was statistically significant(P<0.05).Both groups had incidence of postoperative complications as 5.0%,and the difference was not statistically significant(P>0.05).Conclusion Laparoscopic treatment of acute perforation of gastroduodenal ulcer can shorten the operation time,reduce the length of incision,reduce the incidence of complications.So it is worthy of clinical promotion.
