Performance Status and Efficiency Evaluation of County Public Hospitals in Chongqing City

目的:分析重庆市县级公立综合医院运行状况和效率,为提高医院运行效率提供参考。方法:收集2009-2016年81家重庆市县级公立综合医院运行数据,运用描述性方法分析医院运行变化,运用数据包络分析方法的CCR模型、BCC模型以及Malmquist指数模型进行医院静态和动态效率的变化分析。结果:重庆市县级公立医院资源配置和服务能力得到提升; 81家医院的平均总体效率值为0.860,总体有效医院23家,对非技术有效医院进行管理,可节省大量医疗资源投入; 2009-2016年81家医院的全要素生产率总体处于退步趋势,其中技术变动退步是全要素生产率退步的主要原因。结论:重庆市县级公立综合医院总体效率有待提高,需要合理控制医院规模,加强技术创新,提高医院管理水平,综合提升医院运行效率。

Objective:To analyze the efficiency of county public hospitals in Chongqing municipality and to provide suggestions for efficiency improving and decision-making.Methods:81 hospitals’operating data from 2009 to 2016 were collected,descriptive analysis was used to longitudinally describe the general performances of such hospitals,CCR,BCC and Malmquist index models of DEA method were used to study the static and dynamic changes of hospital operation efficiency.Results:The reform in county public hospitals in Chongqing has achieved success,hospitals resource allocation and service capacity have been improved;the average efficiency score was 0.860,23 hospitals were DEA efficient,a lot of health resources can be saved by managing the non-DEA efficient hospitals;the trend of the total factor productivity change in 81 hospitals from 2009 to 2016 were deteriorated,and the declining trend of technological change was the main reason for the productivity deteriorated.Conclusion:There is still big room for efficiency improvement in sample hospitals.More attention should be paid to control the scale of hospitals,enhance technological innovation and improve hospital management ability to improve the operating efficiency of hospitals.
