自拟益肾生精方治疗少弱精不育症的研究

Study on the treatment with self-made Yishen Shengjing Decoction for infertility due to asthenospermiaand oligospermia

目的观察益肾生精方治疗少弱精不育症患者的疗效。方法将180例少弱精不育症患者随机分为2组,治疗组90例患者采用自拟益肾生精方治疗,对照组90例患者采用左卡尼丁口服液治疗,2组均以3个月为1个疗程。观察2组患者治疗后精液质量和精子DNA损伤改善情况,统计2组患者治疗效果。结果治疗后2组患者精子活率、精子前向运动率、精子前向运动+非前向运动率(PR+NP)、精子计数、精子整体正常形态率均显著提高(P均<0. 05),畸形精子指数、白细胞数及未成熟生精细胞数均显著下降(P均<0. 05),正常形态精子<4%的例数显著减少(P均<0. 05),且治疗组上述指标改善情况均显著优于对照组(P均<0. 05)。2组患者精子总彗星细胞率和Ⅰ级彗星细胞率均显著降低(P均<0. 05),且治疗组均显著低于对照组(P均<0. 05)。治疗组配偶妊娠率为15. 56%(14/90),对照组配偶妊娠率为8. 89%(8/90),治疗组显著高于对照组(P <0. 05)。结论益肾生精方可提高少弱精不育症患者的精子活力和数量,降低精子畸形率,减轻精子DNA损伤程度,改善精液质量,可明显提高配偶妊娠率。

Objective It is to observe the curative effect of Yishen Shengjing Decoction on infertility due to asthenospermiaand oligospermia.Methods 180 patients with infertility due to asthenospermiaand oligospermia were randomly divided into two groups.90 patients in the treatment group were treated with self-made Yishen Shengjing Decoction,and 90 patients in the control group were treated with L-carnitine oral solution.3 months was a course of treatment.The improvement of semen quality and sperm DNA damage in the two groups were observed after treatment,and the therapeutic effects of the two groups were statistically analyzed.Results After treatment in the two groups,the sperm motility,sperm forward motion rate,sperm forward motion+non-forward motion rate(PR+NP),sperm count,and normal sperm morphology were significantly increased(P<0.05),abnormal sperm index and white blood cell count and the number of immature spermatogenic cells were decreased significantly(P<0.05),the number of normal spermatozoa<4%decreased significantly,and the improvements of the above indicators in the treatment group were significantly better than that of the control group(P<0.05).After treatment,the percentage of total comet cells of spermatozoon and grade I comet cells were significantly decreased(P<0.05),and the total sperm comet cell rate and grade I comet cell rate in the treatment group were significantly lower than those in the control group(P<0.05).The pregnancy rate of the spouse in the treatment group was 15.56%(14/90),and the pregnancy rate in the control group was 8.90%(8/90).The pregnancy rate of the spouse in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion Yishen Shengjing Decoction can significantly improve the sperm motility and quantity of patients with infertile due to asthenospermiaand oligospermia,it could reduce the rate of sperm deformity,relieve the degree of sperm DNA damage,improve the quality of semen,and significantly improve the pregnancy rate of spouses.
