药学抗凝门诊建立规范化抗凝管理实例分析

Analysis of the establishment of standardized management of anticoagulation in pharmacy anticoagulation clinic

目的 :建立药学抗凝门诊对患者进行规范化抗凝管理。方法 :通过对2例使用华法林的患者在抗凝门诊随访期间INR异常升高病例的分析,临床药师根据具体情况,寻找原因,并为患者及时调整药物剂量,维持稳定的INR水平。结果 :临床药师对患者进行用药教育并调整华法林剂量,最终达到满意的抗凝治疗效果。结论 :药学抗凝门诊的设立能更好地针对每位患者的具体情况,选择最适合的药物剂量,及时调整抗凝方案,提高患者依从性,极大地减少患者的致残率,降低住院率,减少医疗费用,节约医疗资源,更好地保证抗凝的有效性及安全性。

Objective:To establish a pharmacy anticoagulation clinic to standardize anticoagulation management of patients.Methods:The clinical pharmacist looked for the reason based on the analysis of 2 cases of warfarin-taking patients with abnormal elevation of INR level during follow-up of anticoagulation clinic and their specific situation and adjusted their drug dose in time to maintain a stable INR level.Results:A satisfactory anticoagulant effect was finally achieved by the adjustment of warfarin dose and the medical education.Conclusion:The establishment of pharmacy anticoagulation clinic is helpful to choose the more appropriate dose and anticoagulant method for each patient,which can improve the patient compliance,reduce the disability and hospitalization rate as well as the costs and the medical resources.Meanwhile,the effectiveness and safety of anticoagulation can be guaranteed.
