尿糖、尿微量清蛋白联合检测在糖尿病早期肾损伤诊断中的价值 被引量:7

摘要 目的研究尿糖以及尿微量清蛋白检测在糖尿病(DM)早期肾损伤诊断中的应用价值。方法回顾性分析2015年1月~2017年9月我院收治的DM早期肾损伤患者80例的临床资料,设为观察组;选取同期我院收治的80例DM非肾损伤患者,设为参照组;选取同期我院健康体检中心的80例健康体检人员,设为对照组。对所有人员均进行尿糖和尿微量清蛋白水平检测。结果观察组尿糖、尿微量清蛋白水平高于对照组及参照组,参照组尿糖、尿微量清蛋白水平均高于对照组,差异有统计学意义(P<0.05);观察组尿微量清蛋白阳性率高于参照组,观察组、参照组尿糖阳性率高于对照组,差异有统计学意义(P<0.05);尿微量清蛋白单独检测特异性高于尿糖单独检测,联合检测敏感性、特异性、准确率均高于尿糖、尿微量清蛋白单独检测,差异均有统计学意义(P<0.05)。结论尿糖及尿微量清蛋白检测在DM早期肾损伤诊断中较为敏感。联合检测有利于早期发现DM患者的肾损伤,早期干预治疗,值得临床推广应用。 Objective To study the application value of the detection of urine glucose and urine microalbumin in the diagnosis of early diabetic(DM)kidney injury.Methods The clinical data of80patients with early DM kidney injury who were admitted to our hospital from January2015to September2017were retrospectively analyzed.They were selected as the observation group.At the same time,80patients with DM and non-renal injury who were admitted to our hospital were selected as the reference group.During the same period,80healthy persons who were given health check-up in the Health Examination Center of our hospital were selected as the control group.Urine glucose and urine microalbumin levels were tested for all subjects.Results The levels of urine glucose and urine microalbumin in the observation group were higher than those in the control group and the reference group.The urine glucose level and urine microalbumin level in the reference group were higher than those in the control group,the differences were statistically significant(P<0.05).The positive rate of urine microalbumin in the observation group was higher than that in the reference group.The positive rate of urine glucose in the observation group and the reference group was higher than that in the control group,and the differences were statistically significant(P<0.05).The specificity of single detection of urinary microalbumin is higher than that of single detection of urine glucose alone.The sensitivity,specificity and accuracy of combined detection were higher than those of single detection of urine glucose and urine microalbumin,and the differences were statistically significant(P<0.05).Conclusion Urine glucose and urine microalbumin are sensitive in the diagnosis of early diabetic kidney injury.Combined detection is beneficial for clinicians to detecting early kidney damage in the patients with diabetes,and provide early intervention treatment,which is worthy of promotion.
