HY+CSP治疗中重度盆腔器官脱垂的安全性、疗效及对睡眠质量影响观察

Safety,Efficacy and Effect on Sleep Quality of HY + CSP in the Treatment of Moderate and Severe Pelvic Organ Prolapse

目的:探讨中重度盆腔器官脱垂实施腹腔镜下子宫-阴道骶骨固定术(HY+CSP)治疗的安全性、疗效及对睡眠质量影响。方法:选取2015年1月至2018年1月泉州第一医院收治的对妇科中重度盆腔器官脱垂患者80例,按照随机数表法随机分为对照组和观察组,每组40例,对照组实施阴式子宫切除+阴道前后壁修补术治疗,观察组行HY+CSP治疗,分析总结患者的手术状况、疗效、并发症状况、性生活质量和睡眠质量状况。结果:观察组手术时间、术中出血量、住院时间明显低于对照组(P <0. 05)。观察组术前、术后12个月盆腔Ba、Bp点值与对照组比较,差异无统计学意义(P> 0. 05)。观察组术后未出现并发症,对照组出现腰酸、尿潴留各1例。观察组术后12个月PISQ-12评分明显高于对照组,观察组术后12个月PSQI评分明显低于对照组(P <0. 05)。结论:中重度盆腔器官脱垂实施HY+CSP (腹腔镜下子宫切除+Y型网片阴道骶骨固定术)治疗可达到良好的治疗效果,且手术创伤小,恢复快,可有效改善患者性生活质量及睡眠质量。

Objective:To investigate the safety,curative effect and effect on sleep quality of moderate and severe pelvic organ prolapse treated with(HY CSP)under laparoscope.Methods:From January2015to January2018,80patients with moderate and severe pelvic organ prolapse were selected and randomly divided into control group and observation group with40cases in each group.The patients in the control group were treated with vaginal anterior and posterior wall repair with vaginal hysterectomy,and the patients in the observation group were treated with HY CSP.The operation status,curative effect,complications,sexual quality of life and sleep quality of the patients were analyzed and summarized.Results:The operative time,intraoperative bleeding volume and hospitalization time in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference between the observation group and the control group in Ba,Bp points before and12months after operation(P>0.05).There were no postoperative complications in the observation group,and lumbar acid and urinary retention in the control group(1case each).The PISQ-12score in the observation group was significantly higher than that in the control group12months after operation,and the PSQI score in the observation group was significantly lower than that in the control group12months after operation(P<0.05).Conclusion:The treatment of moderate and severe pelvic organ prolapse by HY CSP(laparoscopic hysterectomy with Y type mesh vaginal sacrum can achieve a good therapeutic effect,and the surgical trauma is small,the recovery is fast,and the quality of life and sleep of the patients can be improved effectively.
