糖尿病视网膜病变患者血浆中miR-21、miR-93的水平变化及临床意义 被引量:6

Changes and clinical significance of plasma levels of miR-21 and miR-93 in patients with diabetic retinopathy

摘要 目的探讨糖尿病视网膜病变患者血浆中微小RNA-21(miR-21)、微小RNA-93(miR-93)的水平变化并分析其临床意义。方法选择糖尿病视网膜病变患者109例(视网膜病变组),单纯糖尿病患者100例(单纯糖尿病组),同期体检的健康人员100例(对照组)。分别检测并比较三组各项生化指标水平,采用实时荧光定量PCR技术检测血浆miR-21、miR-93表达情况。对糖尿病视网膜病影响因素行多因素Logistic回归分析,Pearson相关分析糖尿病视网膜病变患者血浆miR-21、miR-93与各生化指标的相关性。结果视网膜病变组血浆miR-21与miR-93表达水平高于单纯糖尿病组与对照组(P均<0.05)。与单纯糖尿病组及对照组比较,视网膜病变组空腹血糖(FPG)、糖化血红蛋白(HbA1c)、抵抗素、瘦素水平均高,而内酯素水平均低;单纯糖尿病组FPG、HbA1c水平均高于对照组,差异均有统计学意义(P均<0.05)。多因素Logistic回归分析结果显示,FPG、HbA1c、抵抗素、瘦素及血浆miR-21、miR-93均是糖尿病视网膜病的独立危险因素,而内酯素为保护性因素(P均<0.05)。Pearson相关分析结果显示,糖尿病视网膜病变患者血浆miR-21、miR-93与FPG、HbA1c、抵抗素、瘦素均呈正相关,而与内酯素均呈负相关(P均<0.05)。结论糖尿病视网膜病变患者血浆中miR-21、miR-93均明显高表达,二者参与了糖尿病视网膜病变的发展。 Objective To study the changes and clinical significance of plasma levels of microRNA-21(miR-21)and microRNA-R93(miR-93)in patients with diabetic retinopathy.Methods One hundred and nine patients with diabetic retinopathy(retinopathy group),100patients with simple diabetes mellitus(simple diabetic group)and100healthy persons(control group)who underwent physical examination at the same time were selected.The levels of biochemical indicators were detected and compared among the three groups.The expression of plasma levels of miR-21and miR-93were detected by real-time fluorescent quantitative PCR.The influencing factors of diabetic retinopathy were analyzed by multivariate Logistic regression analysis.The correlations between plasma miR-21,miR-93and biochemical index in patients with diabetic retinopathy were investigated.Results The expression plasma levels of miR-21and miR-93were higher in the retinopathy group than in the simple diabetic group and the control group(both P<0.05).Compared with the simple diabetic group and control group,the levels of fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),resistin and leptin in the retinopathy group were higher,while the level of lactone was lower(all P<0.05).The levels of FPG and HbA1c in the simple diabetic group were higher than those in the control group(both P<0.05).The multi-factor Logistic regression analysis showed that FPG,HbA1c,resistin,leptin and plasma miR-21,miR-93were independent risk factors for diabetic retinopathy,and lactone was the protective factor(all P<0.05).Pearson correlation analysis showed that the plasma levels of miR-21and miR-93were positively correlated with FPG,HbA1c,resistin and leptin,but were negatively correlated with lactone in patients with diabetic retinopathy(all P<0.05).Conclusion The plasma levels of miR-21and miR-93in patients with diabetic retinopathy are highly expressed,and both of them are involved in the development of diabetic retinopathy.
