CT在脑疝诊断治疗中的临床应用效果

Clinical application effect of CT in the diagnosis and treatment of cerebral hernia

目的观察CT在脑疝诊断治疗中的临床应用效果。方法选取2015年4月至2018年4月我院收治的71例颅内肿瘤、损伤以及自发性颅内出血并出现脑疝患者的临床及CT资料,对其进行回顾性分析。结果 CT诊断分析结果显示,71例患者中,颅内肿瘤5例(7.04%),颅内损伤37例(52.11%),自发性颅内出血29例(40.85%);脑疝类型:原发性脑疝54例(76.06%),继发性脑疝17例(23.94%)。脑疝类别:大脑镰下疝9例(12.68%),小脑幕切迹疝45例(63.38%),枕骨大孔疝17例(23.94%)。治疗方案选取:非手术治疗18例(25.35%),锥颅血肿冲洗或抽吸13例(18.31%),开颅手术24例(33.80%),侧脑室外引流16例(22.54%)。患者治疗结果:良好18例(25.35%),致残33例(46.48%),死亡20例(28.17%)。结论 CT在脑疝诊断治疗中的临床应用效果显著,其可以尽早发现脑疝情况,并予以相对适宜指导,值得推广。

Objective To observe the clinical application effect of CT in the diagnosis and treatment of cerebral hernia.Methods The clinical and CT data of 71 patients with intracranial tumors,injuries and spontaneous intracranial hemorrhage and all complicated cerebral hernia from April 2015 to April 2018 were retrospectively analyzed.Results The results of CT diagnosis showed that there were 5 cases(7.04%)of intracranial tumors,37 cases(52.11%)of intracranial injuries,and 29 cases(40.85%)of spontaneous intracranial hemorrhage among 71 patients.The types of cerebral hernia showed that there were 54 cases(76.06%)of primary hernia and 17 cases(23.94%)of secondary hernia.The classification of cerebral hernia showed gyri callousus hernia in 9 cases(12.68%),herniation of the cerebellar incisura tentori in 45 cases(63.38%)and tonsillar hernia in 17 cases(23.94%).The treatment options included non-surgical treatment in 18 cases(25.35%),irrigation or aspiration in 13 cases(18.31%),craniotomy in 24 cases(33.80%)and lateral ventricular drainage in 16 cases(22.54%).The results of treatment effects showed that 18 cases(25.35%)were good,33 cases(46.48%)were disabled and 20 cases(28.17%)died.Conclusion The clinical application effect of CT in the diagnosis and treatment of cerebral hernia is remarkable.It can detect the situation of cerebral hernia as early as possible and give relatively appropriate guidance,which is worth popularizing.
