清补地龟汤治疗气虚湿热型慢性肾脏病3期患者随机对照研究

Effect of Qingbudigui decoction on patients of stage 3 chronic kidney disease with qi deficiency and dampness-heat type:A randomized controlled study

目的观察清补地龟汤治疗气虚湿热型慢性肾脏病(CKD)3期的临床疗效。方法将40例气虚湿热型CKD 3期患者1∶1随机分为试验组和对照组,两组均予西医基础治疗,试验组加用清补地龟汤口服;比较6个月后两组血肌酐(SCr)、肾小球滤过率估算值(e GFR)、24 h尿蛋白定量(24 h UP)、中医证候积分及有效率。结果试验组治疗后SCr下降差异有统计学意义(P <0.01),对照组无明显变化(P>0.05),试验组优于对照组(P <0.05);eGFR、24 h UP均无明显变化(P> 0.05)。试验组治疗后中医证候积分下降差异有统计学意义(P <0.05),对照组无明显变化(P> 0.05)。试验组有效率90%,对照组60%,试验组优于对照组(P <0.05)。结论清补地龟汤能够延缓气虚湿热型CKD 3期患者肾脏病进展,改善临床症状。

Objective To observe the clinical effect of Qingbudigui decoction on patients of stage 3 chronic kidney disease(CKD)with qi deficiency and dampness-heat type.Methods Forty patients of stage 3 CKD were randomly divided into test group and control group with 20 cases in each group.The 2 groups were given integrative treatment of CKD,and the test group additionally with Qingbudigui decoction.Serum creatinine(SCr),estimated glomerular filtration rate(eGFR),24 h urine protein excretion(24 h UP),the TCM syndrome scores and clinical effective rate of the two groups 6 months after the treatment were observed and compared.Results SCr in the test group decreased significantly(P<0.01),but that in the control group had no significant change(P>0.05).There was no significant change regarding eGFR and 24 h UP in the 2 groups(P>0.05).The TCM syndrome scores of the test group were improved after the treatment(P<0.05),while those in the control group were not statistically significant(P<0.05).The total effective rate of the test group and the control group was 90%and 60%respectively,which was statistically different(P<0.05).Conclusion The treatment of stage 3 CKD with qi deficiency and dampness heat type by Qingbudigui decoction can delay the progression of the disease and improve clinical symptoms.
