老年首发抑郁与复发抑郁患者的甲状腺功能比较

Thyroid function of first-episode geriatric depression patients versus recurrent depression patients

目的探讨老年首发抑郁症和复发抑郁症患者的甲状腺功能。方法比较47例老年首发抑郁症患者(首发组)、53例老年复发抑郁症患者(复发组)和40名相匹配的正常人(对照组)的血清甲状腺激素水平,测定所有受试者的血清三碘甲状腺原氨酸(T3)、游离三碘甲状腺氨酸(FT3)、甲状腺素(T4)、游离甲状腺素(FT4)水平和促甲状腺激素(TSH)浓度。结果首发抑郁组与正常对照组,复发抑郁组与正常对照组的血清T3、FT3、T4、FT4、TSH水平差异均有统计学意义(P <0. 01或P <0. 05);首发抑郁组与复发抑郁组的血清T3、FT3、T4、TSH水平差异均无统计学意义(P> 0. 05),但2组间FT4水平差异有统计学意义(P <0. 05)。结论老年抑郁症患者的甲状腺功能水平降低,但是老年首发抑郁症与复发抑郁症组间甲状腺功能水平并无显著差异,且治疗前抑郁症状与甲状腺功能水平无相关性。

Objective To investigate the thyroid function in elderly patients with first-episode depression and recurrent depression.Methods Serum thyroid hormone levels were compared in 47 elderly patients with first-episode depression(first-episode group),53 elderly patients with recurrent depression(recurrent group),and 40 matched normal controls.Serum thyroid original glycine(T3),free triiodothyronine(FT3),thyroxine(T4),free thyroxine(FT4)levels,and thyroid stimulating hormone(TSH)concentrations were determined.Results Serum T3,FT3,T4,FT4 and TSH levels in first-episode group versus normal control,recurrent group versus normal control(P<0.05 or P<0.05).There was no significant differences in serum T3,FT3,T4,FT4 and TSH levels between the first-episode depression group and the recurrent depression group(P>0.05),but FT4 level showed significant difference(P<0.05).Conclusion The thyroid function is reduced in elderly patients with depression,but there is no significant difference in thyroid function between the first-episode depression and recurrent depression,and no correlation between depressive symptoms and thyroid function is seen.
