经尿道钬激光与传统电切术治疗非肌层浸润型膀胱肿瘤的疗效比较

Transurethral holmium laser versus conventional electric resection in treatment of non-muscular invasive bladder tumors

目的比较经尿道钬激光与传统电切术治疗非肌层浸润型膀胱肿瘤的疗效。方法将100例非肌层浸润型膀胱肿瘤患者分为2组,激光组采用经尿道钬激光切除术,电切组采用经尿道电切术。比较2组手术指标、并发症发生率、近期治疗效果与远期生存质量。结果激光组手术时间显著长于电切组(P <0. 05),术中失血量、住院时间、导管留置时间均显著优于电切组(P <0. 01)。激光组总并发症发生率、膀胱冲洗率、术后1年内肿瘤复发率显著低于电切组(P <0. 05)。激光组疾病控制率与总有效率显著高于电切组,末期随访时角色领域、情感领域与社会领域评分显著高于电切组(P <0. 05)。结论经尿道钬激光切除术治疗非肌层浸润型膀胱肿瘤的疗效显著,并发症较少,远期生存率较高。

Objective To compare the efficacy of transurethral holmium laser and conventional electric resection in treatment of non-muscular invasive bladder tumors.Methods Totally 100 patients with non-muscular invasive bladder tumors were divided into two groups.The laser group was treated with transurethral holmium laser resection for bladder tumors,while electric resection group was treated with transurethral electric resection.The operation indexes,incidence rate of complications,short-term therapeutic effect and long-term quality of life were compared between the two groups.Results The operation time of laser group was significantly longer than that of electric resection group(P<0.05).The blood loss,hospital stay and catheter indwelling time of laser group were significantly better than those of electric resection group(P<0.01).The total incidence rate of complications,bladder irrigation rate and recurrence rate within 1 year after operation in the laser group were significantly lower than those in the electric resection group(P<0.05).The disease control rate and total effective rate in the laser group were significantly higher than those in the electric resection group,and the scores of role domain,emotional domain and social domain at the end of follow-up were significantly higher than those in the electric resection group(P<0.05).Conclusion Transurethral holmium laser resection for non-muscular invasive bladder tumors is effective with fewer complications and a higher long-term survival rate.
