急性脑卒中患者的院前急救方式探讨

Pre-hospital first aid for patients with acute stroke

目的探讨2组不同院前急救方式对急性脑卒中患者治疗及预后的影响。方法将本院急救中心收治106例急性脑卒中患者随机分为对照组和观察组,各53例。比较2组救治时间、入住专科时间、住院时间、并发症以及死亡率。治疗前后评估2组神经功能情况。结果与对照组比较,观察组院内急救时间、入住专科时间及住院时间均明显缩短(P <0. 05);救治后呼吸衰竭、心力衰竭、神经功能后遗症发生率明显下降,死亡人数明显减少,差异均有统计学意义(P <0. 05)。抢救治疗后,2组患者美国国立卫生研究院卒中量表(NIHSS)评分均明显下降,且观察组NIHSS评分明显低于对照组,差异均有统计学意义(P <0. 05)。结论连贯高效的院前急救能为入院救治争取更多有效时间,从而缩短病程,预防院前院内不良事件发生,降低脑卒中死亡率,显著改善存活患者神经功能,提高患者生存质量。

Objective To explore the effects of different pre-hospital methods on the treatment and prognosis of patients with acute stroke.Methods A total of106patients of acute stroke were randomly divided into control group and observation group.The time to treatment,admission to specialist time,length of hospital stay,complications,and mortality were recorded.The neurological function of the two groups was evaluated before and after treatment.Results Compared with the control group,the emergency time,admission time and hospitalization time of the observation group were significantly shortened(P<0.05).The incidences of respiratory failure,heart failure,and neurological sequelae decreased significantly after treatment,and the number of deaths was significantly less(P<0.05).After rescue treatment,the National Institute of Health Stroke Scale(NIHSS)scores of the two groups were significantly decreased,and the NIHSS scores of the observation group were significantly lower than the control group(P<0.05).Conclusion Optimized pre-hospital emergency treatment could reduce patients′treatment time,improve the prognosis and effectively reduce mortality.
