羟考酮联合舒芬太尼术后静脉自控镇痛对脊柱外科患者免疫功能的影响 被引量:1

摘要 目的分析羟考酮联合舒芬太尼术后静脉自控镇痛对脊柱外科患者免疫功能的影响。方法将2017年1月2018年2月于本院脊柱外科接受治疗的80例患者作为调查对象,随机编号法纳入联合组与对照组各40例。对照组应用舒芬太尼实施术后静脉自控镇痛,联合组术后应用羟考酮联合舒芬太尼实施静脉自控镇痛,比较2组患者的术后视觉模拟评分(VAS)、患者的术后不良反应发生率及患者的免疫功能影响。结果联合组与对照组患者治疗前VAS评分比较差异无统计学意义,治疗后联合组患者的VAS评分显著降低,组间差值比较差异有统计学意义(P<0.05)。联合组患者术后不良反应发生6例,参照组患者术后不良反应发生2例,组间数据比较差异具有统计学意义(P<0.05)。联合组治疗后肿瘤坏死因子(TNF)-α、CD4+、CD4+CD25+、CD4+/CD8+等指标改善程度均优于参照组,组间差异有统计学意义(P<0.05)。结论羟考酮联合舒芬太尼术后静脉自控镇痛,能够显著改善脊柱外科患者免疫功能,缓解患者的疼痛感受,但是患者的治疗期间不良反应发生率较高,需要密切观察患者临床表现。 Objective To analyzed the effect of postoperative intravenous analgesia with oxycodone combined with sufentanil on the immune function of spinal surgery patients.Methods Eighty patients who received treatment in our hospital from January 2017 to February 2018 were selected as the subjects.The random number method was included in the combined group and the reference group(n=40).The reference group applied sufentanil for postoperative intravenous self-controled analgesia.The combined group was treated with intravenous self-controlled analgesia with oxycodone combined with sufentanil after the operation.The postoperative VAS score,the incidence of postoperative ADR and the immune function of the patients were compared between the two groups.Results There was no significant difference in the VAS score between the combined group and the reference group before treatment.The VAS score of the patients in the combined group after treatment was significantly lower,and the difference was statistically significant(P<0.05).There were 6 cases of postoperative adverse reactions in the combined group,and 2 cases in the reference group.There was a statistically significant difference between the two groups(P<0.05).The improvement degree of TNF-alpha,CD4^+,CD4^+CD25^+and CD4^+/CD8^+in the combined group was better than that in the reference group,and there was a significant difference between the two groups(P<0.05).Conclusion Oxycodone combined with sufentanil for intravenous analgesia can significantly improve the immune function of patients with spinal surgery and relieve the pain,but the incidence of adverse reactions is high during the treatment of patients,and the clinical manifestations of patients should be closely observed.
