2型糖尿病合并PDA患者炎性因子与血浆网膜素-1的相关性研究

The correlations between inflammatory factors and plasma omentin-1 in patients with type 2 diabetes mellitus complicated with PDA

目的研究2型糖尿病(T_2DM)合并下肢周围动脉病变(PAD)患者炎性因子与血浆网膜素-1的相关性。方法选取洛阳市第二中医院2015年6月至2016年12月收治的49例单纯T_2DM患者作为对照组,选取同期收治的49例T_2DM合并PAD患者作为观察组,对比两组患者的炎性因子[白细胞介素-6(IL-6)、超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子(TNF-α)水平]以及血浆网膜素-1表达水平的差异性;对观察组患者行踝肱指数(ABI)检测,根据ABI检测结果将观察组患者划分为重症亚组与轻症亚组,对比两亚组间3项炎性因子与血浆网膜素-1的差异性;分析血浆网膜素-1与IL-6、hs-CRP、TNF-α、ABI的相关性。结果观察组IL-6、hs-CRP、TNF-α的表达水平高于对照组,差异有统计学意义(P <0. 05);观察组网膜素-1、ABI低于对照组,差异有统计学意义(P <0. 05)。观察组中重症亚组IL-6、hs-CRP、TNF-α的表达水平高于轻症亚组,差异有统计学意义(P <0. 05);重症亚组网膜素-1、ABI低于轻症亚组,差异有统计学意义(P <0. 05)。网膜素与IL-6、hs-CRP、TNF-α具有负相关性,与ABI具有正相关性(P <0. 05); ABI指数与炎性因子间呈负相关性(P <0. 05)。结论炎性因子的过高表达可使T_2DM合并PDA患者血浆网膜素-1的表达水平异常降低,而这一异常降低与T_2DM合并PDA患者病情恶化有关,应引起临床广泛关注。

Objective To study the relationship between the levels of plasma omentin-1and inflammatory factors in patients with type2diabetes mellitus(T2DM)combined with lower extremity peripheral arterial disease(PAD).Methods A total of49cases of patients with T2DM were selected as control group and49cases of T2DM patients with PAD were selected as observation group from June of2015to December of2016in Luoyang the Second Hospital of A.C.M.Inflammatory factors were compared between the two groups:interleukin-6(IL-6),high sensitive C reactive protein(hs-CRP),tumor necrosis factor-a(TNF-a)and plasma omentin-1expression.The ankle brachial index(ABI)of the patients in observation group were all detected,and the patients were divided into severe subgroup and mild subgroup according to the results of ABI.The differences were compared in the two subgroups between three inflammatory factors and plasma omentin-1.The correlations were analyzed between plasma omentin-1and IL-6,hs-CRP,TNF-α,ABI.Results The expression levels of IL-6,hs-CRP and TNF-αin the observation group were significantly higher than those in the control group(P<0.05),and the level of omentin-1and ABI in the observation group were significantly lower than that in the control group(P<0.05).The expression levels of IL-6,hs-CRP and TNF-a were significantly higher in the severe subgroup than in the mild group(P<0.05),and the omentin-1and ABI in the severe subgroup were significantly lower than that in the mild group(P<0.05).Omentin-1had significant negative correlation with the IL-6,hs-CRP,TNF-a,and it had significant positive correlation with the ABI(P<0.05).ABI had significant negative correlation with inflammatory factor(P<0.05).Conclusion The high expression of inflammatory factors in the patients with type2diabetes mellitus patients with PDA expression level of omentin-1is abnormally low,and this abnormal decrease of T2DM combined with PDA in patients with disease progression.It should be paid more attention in the clinic.
