慢性萎缩性胃炎肠上皮化生中医证型与腺管开口形态的相关性研究

The Correlation between Different TCM Syndromes of Intestinal Metaplasia of Chronic Atrophic Gastritis and the Open Morphology of Glandular Canal

目的:探讨慢性萎缩性胃炎肠上皮化生不同中医证型与腺管开口形态的相关性。方法:将244例慢性萎缩性胃炎肠化病例进行中医证候分类,观察放大染色内镜下腺管开口形态。结果:A型腺管开口在肝胃不和证组比率(62.5%)较其他组高(?2=91.5143,P=0.00<0.05);B型腺管开口在湿热内蕴证组比率(67.27%)较其他组高(?2=153.6636,P=0.00<0.05);F型腺管开口在胃阴不足证组比率(41.86%)较其他组高(?2=292.7127,P=0.00<0.05)。结论:肝胃不和证为萎缩性胃炎发病早期证型,应早期给予干预;发病中期多属于湿热内蕴证;发病后期则属于瘀血阻络证;表明腺体开口与中医证型存在一定的关系。

Objective:To explore the correlation between different Traditional Chinese medicine(TCM)syndromes of intestinal metaplasia of chronic atrophic gastritis and the open morphology of glandular canal.Methods:A total of244cases of chronic atrophic gastritis were classified according to TCM syndromes,their open morphologies of glandular canal under magnification staining endoscopy were observed.Results:The ratio(62.5%)of type Agland duct openings in the hepato-gastric disharmony syndrome group was higher than that of other groups(x^2=91.5143,P=0.00<0.05).The ratio(67.27%)of type B glandular openings in the damp-heat accumulation group was higher than that of other groups(x^2=153.6636,P=0.00<0.05).The ratio(41.86%)of type F gland duct openings in the gastro-insufficiency group was higher than that of other groups(x^2=292.7127,P=0.00<0.05).Conclusion:The syndrome of liver and stomach disharmony is the early syndrome of atrophic gastritis,which should be intervened at the early stage.The middle stage of the disease belongs to damp-heat syndrome.In the late stage of the disease,it belongs to the syndrome of blood stasis blocking collaterals.It indicates that there is a certain relationship between glandular openings and TCM syndromes.
