学龄儿童幽门螺杆菌感染状况及危险因素分析 被引量:5

摘要 目的通过调查学龄儿童幽门螺杆菌(Hp)感染状况,分析Hp相关危险因素。方法选取2015年5月—2018年5月于我院就诊且伴有腹痛腹胀等临床症状的学龄儿童625例(其中521例参加13C呼气试验)作为调查对象,分析病例Hp感染情况,采用问卷调查的方式,记录病例日常居住环境及生活习性,家族胃病史等内容,对Hp感染的危险因素进行分析总结。结果 Hp感染率为79.85%; Hp感染与性别、喜爱零食无相关性(P> 0.05),与地区、年龄、胃病史、父母文化程度、家庭年收入、常啃手指、漱口杯公用存在相关性(P <0.05);Logistic回归法分析结果发现,农村地区、家庭年收入<10万元、年龄13~14岁、有胃病史、常啃手指、漱口杯公用、父母文化程度为大专以下是影响Hp感染的危险因素。结论家居环境、生活及饮食习惯、家族病史、年龄、经济状况均是影响学龄儿童Hp感染的危险因素,针对学龄儿童学校、家庭、社区等多个支点应该加强Hp感染的教育宣传工作,达到预防的目的,改善学龄儿童生活质量。 Objective To analyze the risk factors of Helicobacter pylori(Hp)infection in school-age children by investigating the infection status of Helicobacter Pylori.Methods 625 school-age children(521 of them participated in the 13C breath test)with abdominal pain and abdominal distention who were hospitalized in our hospital from May 2015 to May 2018 were selected as the subjects.The daily living environment,living habits and family history of gastropathy were recorded by questionnaire.The risk factors of Hp infection were analyzed and summarized.Results The infection rate of Hp was 79.85%.There was no correlation between Hp infection and sex or snack preference(P>0.05),but there was correlation between Hp infection and region,age,history of stomach disease,educational level of parents,annual family income,frequent biting of fingers and common use of mouthwash cups(P<0.05).Logistic regression analysis showed that the risk factors of Hp infection were rural area,family annual income less than 100 thousand yuan,age with 13~14 years old,gastric disease history,frequent biting of fingers,gargle cup for public use and parental education below the specialty level.Conclusion Home environment,living and eating habits,family history,age and economic status are all the risk factors of HP infection in school-age children.Education and propaganda of HP infection should be strengthened in school,family,community and other fulcrums for school-age children to achieve the purpose of prevention and improve the quality of life of school-age children.
