急性胰腺炎的多层螺旋CT表现及病因分析

Multi-slice Spiral CT Findings and Clinical Etiological Analysis of Acute Pancreatitis

目的探讨急性胰腺炎的多层螺旋CT影像学表现及相关临床发病特征。方法回顾性分析2015年6月—2017年8月来我院就诊的46例确诊的急性胰腺炎患者多层螺旋CT及临床资料。采用64排多层螺旋CT扫描,观察胰腺形态、渗出、积液、出血坏死及并发症情况。结果 46例急性胰腺炎病例中,最常见的CT表现为胰腺周围、肠系膜、大网膜、横结肠系膜内渗出,共40例(87%);急性水肿性胰腺炎38例,出血坏死性胰腺炎8例。并发症:胸腔积液21例,胰腺周围脓肿1例,腹腔积液6例,静脉血栓1例,脂肪肝5例。女性中胆源性胰腺炎是最常见类型,占70%,男性中酒精性胰腺炎最多见,占42%。结论多层螺旋CT扫描对于急性胰腺炎的诊断和严重程度评价具有重要价值。

Objective To investigate the multi-slice spiral CT imaging manifestations and clinical features of acute pancreatitis.Methods The multislice spiral CT and clinical data of 46 patients with acute pancreatitis diagnosed in our hospital from June 2015 to August 2017 were analyzed retrospectively.The pancreatic morphology,exudation,effusion,hemorrhage,necrosis and complications were observed by 64 row multislice spiral CT scanning.Results Among 46 cases of acute pancreatitis,the most common CT manifestations were peripancreatic,mesenteric,omentum,transverse mesocolic exudation(87%).There were 38 cases of acute edematous pancreatitis and 8 cases of hemorrhagic necrotizing pancreatitis.Complications:pleural effusion in 21 cases,peripancreatic abscess in 1 case,peritoneal effusion in 6 cases,venous thrombosis in 1 case,fatty liver in 5 cases.Biliary pancreatitis was the most common type in women,accounting for 70%,and alcoholic pancreatitis was the most common type in men,accounting for 42%.Scanning is of great value in the diagnosis and severity evaluation of acute pancreatitis.Conclusion Multislice spiral CT scan is of great value in the diagnosis and severity evaluation of acute pancreatitis.
