胆囊十二指肠瘘的临床诊治效果分析

Analysis of the clinical diagnosis and treatment of cholecysto duodenum fistula

目的分析胆囊十二指肠瘘的临床诊治效果。方法回顾性分析我院31例胆囊十二指肠瘘患者的临床诊治情况。结果 31患者术前行CT扫描9例确诊为胆囊十二指肠瘘,诊断率为29.03%,术中探查确诊均为胆囊十二指肠瘘。31例患者均采用腹腔镜下胆囊十二指肠内瘘修补术进行治疗,有3例因病情特殊中转开腹。平均手术时间(167.34±28.27)min,拔管时间(38.34±5.11)h,住院时间(16.37±6.29)d;术后有1例患者出现腹腔出血,1例患者出现腹腔感染,无胆瘘、十二指肠瘘等并发症,并发症发生率为6.45%。结论胆囊十二指肠瘘患者采用腹腔镜修补术治疗效果良好,围术期相关指标较优,并发症发生率低,具有较高的安全性,可有效促进患者术后恢复。

Objective To analyze the clinical diagnosis and treatment of cholecysto duodenal fistula.Methods The clinical diagnosis and treatment of31patients with gallbladder and duodenal fistula were retrospectively analyzed.Results9cases were diagnosed as cholecystoduodenal fistula by CT scan before operation,the diagnostic rate was 29.03%.All cases were diagnosed as cholecystoduodenal fistula by intraoperative exploration.All 31 patients were treated with laparoscopic cholecystoduodenal fistula repair,and3patients were converted to laparotomy due to special conditions.The average operation time was(167.34±28.27)minutes,the extubation time was(38.34±5.11)hours,and the hospitalization time was(16.37±6.29)days.Postoperative abdominal hemorrhage occurred in1patient,abdominal infection in1patient,no biliary fistula,duodenal fistula and other complications,the incidence of complications was6.45%.Conclusion Laparoscopic repair of cholecystoduodenal fistula has a good therapeutic effect,the perioperative related indicators are better,the incidence of complications is low,has a high safety,can effectively promote the postoperative recovery of patients.
