摘要
目的分析孕妇血清储备铁对糖耐量的影响,及其与妊娠糖尿病病因学间的相关性。方法选择临床明确诊断为妊娠糖尿病且不合并贫血的孕妇362例为试验组,另选取同一时段分娩的正常孕妇365例为对照组。回顾性分析纳入孕妇的一般资料,记录所有孕妇在孕第12周检测的血常规值及血清铁水平,以及孕第24~28周的糖耐量试验结果。比较两组孕妇早孕期血常规情况及血清铁蛋白水平,研究两组孕妇早孕期血清铁蛋白含量与其进行糖耐量试验时服糖1 h及2 h后血糖间的相关性。结果试验组孕妇早孕期血清铁蛋白含量明显高于对照组[(83. 7±57. 8)μg/L比(61. 7±42. 3)μg/L,P <0. 01];试验组孕妇早孕期血清铁蛋白含量与其服糖1 h(r=0. 214,P=0. 039)及2 h(r=0. 432,P <0. 01)后血糖水平均呈正相关,且差异有统计学意义(P <0. 05)。结论孕妇早孕期血清储备铁可能会影响孕妇的糖耐量,甚至可能与妊娠糖尿病的发病相关。
Objective To analyze the effect of serum iron store on glucose tolerance in pregnant women and its correlation with etiology of gestational diabetes mellitus( GDM). Methods Totally 362 pregnant women clearly diagnosed GDM without anemia were selected as the experimental group. Another 365 normal pregnant women during the same period were selected as the control group. The general data of pregnant women were retrospectively analyzed. The blood routine values and serum iron levels of all pregnant women during the 12 th week of pregnancy were recorded,and the results of OGTT test from the 24 th week to 28 th week of pregnancy were recorded. The routine blood test and the serum iron level between the two groups were compared,the correlation between the serum iron level and the blood glucose one hour or two hours after drinking the sweet water in the OGTT test separately were analyzed. Results The serum iron level of pregnant women in the early pregnancy of the experimental group was higher than that of the control group [( 83. 7 ± 57. 8) μg/L vs.( 61. 7 ± 42. 3) μg/L,P < 0. 01 ]. The serum iron level was positively correlated to the blood glucose one hour( r = 0. 214,P = 0. 039)and two hours( r = 0. 432,P < 0. 01) after drinking the sweet water in the OGTT test separately,and the difference was significant( P < 0. 05). Conclusion The serum iron store in the early pregnancy could probably affect the pregnant women’s glucose intolerance,even lead to GDM.
