2型糖尿病患者血清生长分化因子-15水平与心血管病风险的关系

Serum level of growth differentiation factor-15 in type 2 diabetes mellitus and its relation to cardiovascular risk

目的探讨2型糖尿病(T2DM)患者血清生长分化因子(GDF)-15与心血管病风险的关系。方法选择107例糖尿病患者,根据是否合并颈动脉斑块分为A组(单纯T2DM)69例,B组(T2DM合并颈动脉斑块)38例,另选择29例体检健康中年人作为正常对照组C组,记录年龄、性别、身高、体重,吸烟状态、血压、计算体质量指数(BMI)、弗兰明汉风险评分(FRS),检测血清GDF-15、血脂。B超检测颈动脉有无斑块。对检测结果进行统计学处理,分析其影响因素。结果 A组及B组血清GDF-15水平、FRS、年龄、体重指数、三酰甘油均高于C组(P<0.05);B组血清GDF-15水平、年龄、舒张压、FRS高于A组(P<0.05);线性回归分析显示GDF-15水平与年龄、颈动脉斑块、FRS呈正相关(P<0.01),在校正年龄和存在颈动脉斑块与否后,血清GDF-15水平和FRS呈正相关(P<0.01)。结论血清GDF-15在2型糖尿病患者中心血管病风险呈独立相关,是较传统心血管病风险评估指标更有效的临床生物标记物。

Objective To investigate the level of growth differentiation factor15(GDF-15)in the serum of patients with type2diabetic mellitus(T2DM)and its relation to cardiovascular risk.Methods The107patients with diabetes according to whether the merger of carotid plaques were divided into group A(pure T2DM,69cases)and group B(T2DM combined carotid plaques,38cases),the29cases of medical healthy middle-aged were enrolled as normal control group(C),for recording age,gender,height,weight,smoking status,measuring blood pressure,calculating body mass index(BMI),Fleming han(FRS)risk score,detecting serum GDF-15,blood lipid;B ultrasound was used to detect the presence of carotid plaque.Results Serum GDF-15,FRS,age,body mass index and triglycerides were higher in group A and group B than those in group C(P<0.05).Serum GDF-15level,age,diastolic blood pressure and FRS of group B were higher than group A(P<0.05).Linear regression analysis showed that the GDF-15level was positively correlated with age,carotid plaques(P<0.01),after adjustment for age and carotid plaques or not,serum level of GDF-15and FRS were positively correlated(P<0.01).Conclusion Serum GDF-15is an independent factor of cardiovascular risk in patients with type2diabetes mellitus,which suggest that the serum level of GDF-15may be an useful clinical biomarker better than the traditional cardiovascular risk score in type2diabetes mellitus patients.
